Enclosed please find documents provided by the Virginia Department of
Health (VDH) in response to your Virginia Freedom of Information Act request
submitted to VDH dated August 7, 2018.

The following documents are enclosed:

· All emails regarding the state-mandated ultrasound list between VDH and

ultrasound providers from January 10, 2017 to present.
· An excel spreadsheet of ultrasound survey data currently stored in

SurveyGizmo; a data collection tool used by VDH.
Please advise if you require anything further in reference to our response to your
FOIA request.

Hello Ms. Long,

This email acknowledges receipt of your Virginia Freedom of Informabon Act (email) request dated August 7, 2018.

Because of the bme required to search for and copy the records requested, it is not pracbcally possible for VDH to
respond to your request within five working days of receipt of the request. Therefore, pursuant to § 2.2-3704(B)(4)
of the Code of Virginia, VDH is invoking a seven work day extension allowed by that secbon of the Code. VDH’s
deadline for responding to your request is now August 23, 2018.
