The authors are a senior staff radiologist and a junior staff radiologist from a university hospital, both specializing in musculoskeletal imaging, and a radiologist working in private practice with close contacts with academic radiology and a keen interest in teaching. All of them have worked in several institutions in different countries. This guarantees a broad view on the topic and adds details not covered by other textbooks. This includes a discussion of nerves and ves- sels about the knee. The authors’ background also guarantees practical rele- vance of the covered topics. An emphasis on postoperative imaging is another interesting aspect of this book which is not as widely covered in literature as anatomy, variants, acute and chronic trauma or cartilage abnormalities. The book is characterized by a very systematic approach to disease, which is rel- evant for teaching, reliable reporting, quality initiatives and research purposes. MRI of the Knee – A Guide to Evaluation and Reporting is a carefully edited book covering all relevant aspects of MR imaging of the knee, which will fi nd its way to the bookshelves of many radiologists and clinicians inter- ested in the knee. Zurich , Switzerland Jürg Hodler v Foreword II MRI of the Knee – A Guide to Evaluation and Reporting focuses on providing a strong platform for learning and understanding the standard evaluation and accurate reporting of knee MR imaging. The authors, experienced in the fi eld of musculoskeletal imaging, have foreseen the practical fi t of the thorough approach depicted in this textbook for both radiologists – in assessing MR images of the knee systematically – and referring specialists, who will become more familiar with the terminol- ogy used in the reports covering various conditions. The book is structured into 11 chapters, with each chapter covering the normal anatomy and normal MR imaging fi ndings, followed by a detailed description of abnormal and postoperative fi ndings well supported by rele- vant case images. E ach chapter ends with a summary written in the form of a paragraph that readers are encouraged to use as a standardized MR report. MRI of the Knee – A Guide to Evaluation and Reporting is bringing a unique and distinctive approach to knee imaging, with practical application related to day-to-day reporting, educational purposes such as teaching and further research into this part of the musculoskeletal system mostly benefi ting from the state-of-the-art MR imaging evaluation. New York, USA John A. Carrino , MD, MPH vii Pref ace In the past few years, magnetic resonance (MR) imaging of the knee has become the current state-of-the-art imaging modality to evaluate knee disor- ders. Decreasing costs and the noninvasive nature of MR examinations made them widely accepted for the evaluation of meniscal and ligamentous as well as bone marrow and soft tissue injuries or abnormalities. In current medical practice, MR examination is essential not only in the preoperative setting but in general for the fi rst-line diagnosis of most knee derangements. Theoretically, patient history and the course of an accident are often strongly suggestive of certain knee injuries and could be examined clinically. Practically, the rela- tively low sensitivity of clinical examination and the fact that clinical exami- nation is often hampered by pain make diagnostic imaging indispensable. In the past, diagnostic arthroscopy has played an important role. However, since the introduction of MR imaging of the knee, this rather invasive diagnostic tool has also widely been replaced by MRI. Moreover, MR imaging has been increasingly used for detailed preoperative planning. Today’s importance of MR imaging of the knee is also illustrated by the fact that apart from the spine, more musculoskeletal MRI examinations are performed on the knee than on any other region of the body. W ith the widespread use of an imaging technique, several challenges in terms of quality arise. Firstly, image acquisition must follow current stan- dards, and image quality has to be provided on a high diagnostic level. Secondly, image storage and transfer need to be guaranteed to supply refer- ring physicians with the original source information of the examination. Lastly, image evaluation and reporting of fi ndings have to meet the referring physician’s need and must answer the medical question. Therefore, standard- ized evaluation and reporting were suggested by several imaging societies and were proposed as a potential solution. This background led PD Dr. Gustav Andreisek and Dr. Nicolae Bolog to aim for a book project, which was dedicated to the idea to provide the educa- tional basis for standardized evaluation and reporting of knee MR imaging. The book was created as a classic textbook, with several chapters ordered by anatomical structures. Each chapter contains a detailed description of the nor- mal anatomy, normal and abnormal MR imaging fi ndings as well as postop- erative fi ndings. At the end of each chapter, a summary is provided in the form of a paragraph that can be used for a standardized radiological report. ix
