beencompleted m substantialaccordancewiththeterms andconditionsoftheauthorization. Form No.
618, NoticeofCompletionofChangeofAppropriationWaterRight, must becompletedandsubmittedto
the Departmentonorbeforethecompletiondeadlinedateindicatedontheauthorization. Iftheautho-
rizedchangecannotbecompletedbeforethedeadline, anextensionoftimemayberequestedbyfiling
FormNo. 607,ApplicationforExtensionof Time.
FormNo 618mustbecompletedbyapersonexperienced inthedesign,construction,oroperationofthe
appropriationworks. This mayinclude,but isnot limitedto, hydrologists,waterresourceconsultants,
irrigationspecialists, engineers,orthe authorization holderifexperiencedinwatermeasurementorlegal
landdescriptions. Iftheform isnotcompletedproperlyorcorrectedwithinthetimeallowedbytheDe-
partment, theauthorization maybesubjecttorevocationpursuanttoSection85-2-402, MCA.
FormNo. 618istobecompletedwithinformationshowinghowthechangewasdeveloped. Whencompleting
FormNo. 618. followtheline-by-lineinstructionsbelow. Ifyouhaveanyquestions,contactyourlocalWaterRe-
sourcesRegionalOfficelistedonthebackpage.
GeneralInformation
Listthe AuthorizationtoChangenumber. The numberislistedontheauthorizationorca—nbe
foundin DNRC—'s records. Mostauthorizationnumbershavefourparts:two lettercodes one in
parentheses anumericpart, andawaterbasin number. (Example:G (VJ) 123455 -43Q) Some
maynothavetheletterinparentheses. Completethedatetheauthtorizationwas issued. This
datecanbefoundonthebottomoftheauthorization.
Listthe nameofthecurrentowneroftheauthorization, mailingaddress, anddaytimephone num-
ber. Ifthecurrentownerisdifferentthantheownerlistedontheauthorizationorinthe DNRC
records, aWater RightTransferCerlificate FormNo. 608mustbefiledtoupdatethe DNRCown-
ershiprecords.
Ifthefieldexaminer(thepersoncompletingthisform) issomeoneotherthantheauthorization
holder, indicatethatperson'sfull name,professionaltitle, mailingaddress, anddaytimephone
number. Ifthefieldexamineristheauthorizationholderwrite"same." Indicatethedateonwhich
theprojectwasexamined.
