Foreword ACSM’s Resource Manual for Guidelines for Exercise Testing and Prescription, first published in 1988, was developed to be a comprehensive, explanatory companion text to ACSM’s Guidelines for Exercise Testing and Prescription. After much consideration and research, ACSM’s Publications Committee, with the support of ACSM’s Committee on Certification and Registry Board (CCRB), determined that there was a need to reenvision the Resource Manual to meet the evolving needs of the field. Out of this strategic planning, two resources that could be paired with Guidelines in a classroom setting were developed: this book — ACSM’s Exercise Testing and Prescription — and another book (ACSM’s Clinical Exercise Physiology). We couldn’t have selected better editors than the two individuals who led this project. Madeline Paternostro Bayles and Ann M. Swank brought together leading experts in the field to present the most accurate and up-to-date information. You’ll find they have provided the latest scientific information in accordance with the standard set forth by ACSM’s Guidelines for Exercise Testing and Prescription. We hope you will be as pleased as we are with their work and find ACSM’s Exercise Testing and Prescription, first edition, to be an asset to your toolbox. Jeffrey A. Potteiger, PhD, FACSM Chair, ACSM’s Publications Committee Dean, The Graduate School Authorizing Institutional Official Research Integrity Officer Grand Valley State University Grand Rapids, Michigan Preface This first edition of ACSM’s Exercise Testing and Prescription is geared toward undergraduate students studying exercise physiology as well as those seeking ACSM certifications. This book will be a useful resource for those wishing to translate the knowledge of exercise into practice. ACSM’s Exercise Testing and Prescription is based on the most up-to-date information provided in ACSM’s Guidelines for Exercise Testing and Prescription, 10th edition. Organization This book is divided into four separate parts to assist the reader in navigating the deeper knowledge base within each area. Part I: Physical Activity and Health covers the description, the recommended amounts, and the benefits of physical activity as well as the detrimental impact of inactivity. Part II: Preparticipation Screening, Fitness Assessment, and Interpretation provides the latest of the preparticipation health screening recommendations and health-related components of fitness (cardiorespiratory fitness, muscular fitness, body composition, and functional and flexibility assessments). Part III: Exercise Prescription covers the general principles of exercise prescription and provides guidelines for three stages that occur across the lifespan (pregnancy, children and youth, and older adults. Part IV: Exercise Testing and Prescription for Special Populations refines the exercise testing, prescription, and progression information for a variety of special populations beyond the content presented in ACSM’s Guidelines for Exercise Testing and Prescription, 10th edition. Features Parts I–III emphasize educating the reader regarding a variety of topics important to the health and fitness practitioner from physical activity to screening to prescription. Chapters 9–23 include frequency, intensity, time, and type (FITT) tables, which expand on the FITT principle presented in ACSM’s Guidelines for Exercise Testing and Prescription, 10th edition. Each chapter within Part IV presents a real-world case study selected specifically for the special population to bridge the gap between science and practice and also provide contextual framework for practitioners in working with each of these special populations. Case study questions are integrated throughout these chapters to guide the reader through assessment and prescription. Additional Resources ACSM’s Exercise Testing and Prescription includes additional resources for students and instructors that are available on the book’s companion Web site at http://thepoint.lww.com. Students can access: Online Case Studies Approved adopting instructors will be given access to the following resources: PowerPoint lecture outlines Test Generator Image Bank, including all figures and tables from the text See inside the front cover of this text for more details, including the passcode you will need to gain access to the Web site. Acknowledgments The authors wish to thank members of the ACSM’s Committee on Certification and Registry Board (CCRB) for the original concept of this book as well as the ACSM’s Publications Committee, Greg Dwyer, Jeffrey A. Potteiger, and, in particular, Dierdra Bycura, for the effort they took to get this diverse publication reviewed. The hardworking individuals from both these committees are tireless in their efforts to produce excellent work that represents the ACSM. Thank you to Deb Riebe and coauthors of ACSM’s Guidelines for Exercise Testing and Prescription, 10th edition (GETP10) for always accepting my calls and for producing a book of such high standards and quality which served as a guide in the development of this publication. Finally, thank you to the individual chapter authors. Thank you for the high-quality work that we asked you to create and the attention you paid to making sure that this book is a useful tool for all students and exercise professionals. Additionally, this publication could not have come to a completion without the patience (lots of it) and guidance of Katie Feltman, ACSM Chief Content Officer, and Michael Nobel, WK Executive Editor. Additional gratitude and thanks to Amy Millholen, WK Senior Product Development Editor; Robin Bushing, Developmental Editor; and Angela Chastain, ACSM Editorial Assistant; for their enormous display of patience and hard work in working with Ann and I through this process. From Madeline Paternostro Bayles: On a personal note, I want to thank my family, son, and daughter for their support and especially my husband, Bob, to whom I promised not to say yes to writing anything else. Also, my IUP colleagues who put up with me during this process. Also, this book is dedicated to the memory of an extraordinary person and member of the CCRB, Dr. Teresa Connelly Fitts, FACSM — gone too soon, but an inspiration to all who knew her. From Ann M. Swank: On a personal note, I would like to thank my family (every one of them) and my colleagues for helping me through this very worthwhile project. I am very happy to have this textbook as my last professional activity before retiring. I would also like to thank the entire staff of ACSM and WK for their patience and strong work ethics and professionalism in completing this project. It has been worthwhile.
