Please read the following instructions carefully so thatyou will understand how to register for next semester. Registration Forms will not be accepted after April 30, 1993. WHERE TO OBTAIN YOUR REGISTRATION FORMS 1. CAS declared majors at major departments. 2. CAS Undeclared majors should go to the University Advising Center, McCormack Building, 3rd floor. Room 625. 3. CPCS registration forms can be picked up outside the CPCS Academic Affairs Office, Wheatley Building, 4th floor, or from the Wheatley Registration Center, Wheatley, 1st Floor. Be sure to obtain both course and competency registration forms. 4. Management majors at University Advising Center, McCormack Building, 3rd floor. Room 625. 5. FORMS MUST BE RETURNED TO THE APPROPRIATE OFFICE BYAPRIL 30, 1993. • Continuing students who do notsubmitregistration forms by April 30, 1993will beassesseda$50 late registration fee. The nextavailable time a continuing student can register for Fall 1993 will be Add/Drop period. • CPCS students will turn in their forms directly to the Wheatley Registration Center once they have obtained their instructors’ and faculty adviser’s signature on the course and competency registration forms. REGISTRATION FORMS 1. Each student is given a form on which to register. This form allows the option of registering for nine courses or seven courses with two alternates. CPCS students—should follow the instructions in the CPCS Instructional Activity Book and their Red Book. 2. Free Time Exempt Form These forms are used so thatstudents may indicate particular hours during which they do notwish classes to be scheduled. To insure thatthe information can be interpreted, it is essential thata #2 pencil be used to fill in the slots corresponding to the times and days of the week during which you do notwant classes scheduled. Use ofthese forms does not enhance your chances of receiving a section thatyou request, and could in fact limityour ability to obtain a course because you will not be assigned to sections that meet at hours from which you have exempted yourself. Use of this form could limityour chances of receiving a full schedule. 3. The registration form and time exemption form are read by machine. This means the markings must be completed carefully, i.e., no erasures, stray marks, folds, etc. A #2 pencil must be used. SEMESTER SCHEDULE BtDOKLETS 1. The schedule booklet is the list of courses that will be offered in the Fall 1993 semester. Your program for the next semester should be chosen from this booklet, alongwith the course addendawhich is a list ofchanges made to the semester booklet (added sections, cancelled 3. courses, etc.). Please do not selectcourses from the schedule bookletwithout checking the addendum as changes may have been made. 2. CPCS students can obtain the CPCS Instructional Activity Book and addenda from Academic Affairs, Wheatley, 4th floor. The UniversityBulletin is a complete listing of courses offered with descriptions. University requirements are also listed in this book. Be sure that all numbers are penciled neatly and completely. Double check to see that entries are accurate. If a number is incorrect or illegible or not blocked in totally, the entire transaction will be invalidated. 2. They may not be cancelled or refunded unless studentwithdraws prior to the first day of classes. ‘‘Commencement Fee: Compulsory one time $40.00 fee assessed of seniors or students in the year of their anticipated graduation. ‘"Yearbook. The $20.00 fee is mandatory and billed to seniors. It is optional foreveryone else and may be added to the bill at the time of bill payment. ‘"‘Special Student Registration Fee: Mandatory fee charged to each non-matriculated student each semester. Curriculum Support Fee: Due to serious budgetreductions, the Board the Trustees mandated this fee for all students on a semester by semester basis to pay for instructors, advisors, student scholarships and equipment essential for educational programs currently in place. Late Fees: $50.00 charge for all students with a late registration determined by the registrar and/or late bill payment of bill clearance. CANCELLATION AND REFUND OF TUITION AND FEES Submission ofa registration form and acceptance Into the courseIncurs a financial obligation for thatcourseexceptas follows: A studentwho completely withdraws from the university before the sixth week of the semester will be granted a percentage refund of tuition and some fees based on refund schedule. (Failure to attend does notconstitute awithdrawal.) A withdrawal form must be completed by the student, A studentwho submits awithdrawal form, prior to the start of the semester at the university, will be given a full refund of tuition and fees. Afterthe add/drop period no refundwill be given for course load reductions. Studentswithdrawing completely from the university during the first five weeks of a semester and receiving grades of "W” for all courses will receive partial refunds. After five weeks no other refunds will be issued if any grades including "W”, “NA”, or INC. are awarded.
