The volume dealt with the pathology of those diseases which occur mainly or ex clusively in the tropics and subtropics. In the preface to the first edition the aim of the book was characterized as follows: "The book is designed as a reference work for pathologists and clinicians working in tropical regions and faced with diagnostic problems". In addition some aspects of tropical diseases were analys ed in Vol. 5 of Spezielle pathologische Anatomie, which was published in 1966 and contained not only the pathological anatomy of Mediterranean and tropical diseases (edited by Professor B. MAEGRAITH), but also the principal features of geographical pathology (edited by Professor F. HENSCHEN). In the last two decades a good deal of new information has been collected and new technologies have been established for the diagnosis of tropical diseases. The continuous and rapid expansion of tourism in all countries of the world and the increasing emigration of peoples of the developing countries to the "old continents" have changed the classical image of tropical diseases. The truly tropical diseases are seen more frequently in temperate climates and highly developed countries. For example, HIV-related diseases are no longer an endemic African infectious entity, but a world-wide problem. Therefore, knowl edge of primary tropical diseases has become even more important: some tropical diseases are even cosmopolitan in their distribution in the meantime. Hence, a new conception was necessary for the second edition of the Tropi cal Pathology. The presentation of the material has been complemented by new chapters which reflect the new developments. The chapter "Guidelines for Autopsies in the Tropics" which was written by Professor SEBASTIAN LUCAS (Department of Histopathology, University College, London Medical School) is very instructive regarding the role of the autopsy, also discussing safety issues, especially of the risks of acquiring infection from an autopsy in hepatitis, tuber culosis or HIV. A further aspect is the integration of a chapter on geomedicine written by Professor Dr. H. J. DIESFELD (Institute of Tropical Hygiene of the University of Heidelberg), under scoring the importance of this discipline for tropical dis eases. The geography of health is closely associated with the pathology of developing countries and the living conditions there, and so geomedicine deals not only with tropical diseases, but also with geographical ecological aspects. Tropical diseases are influenced by social determinants so that geomedicine estimates the burden of illness in the tropics. VI Preface The chapter "Genetic Diseases in the Tropics" which was written by Professor T. JENKINS (Department of Human Genetics, South African Insti tute for Medical Research and University of Witwatersrand, Johannesburg/ South Africa) offers new insights into genetic disorders under consideration of diseases in the developing countries. Inherited disorders are assuming greater relative importance in many countries, including some in the tropics. The chapter "New Technology to the Diagnosis of Infectious Diseases" by Dr. A.M. MARTY and Dr. S. R. ZAKI (Armed Forces Institute of Pathology in Washington and Centers for Disease Control in Atlanta, USA) is an important introduction to the chapters which deal with such different topics as bacterial, viral, protozoal or mycotic diseases. The section on the special pathology of organ systems was extended by the chapter "Renal Diseases in the Tropics", written by Professor R. SINNIAH, Singapore and Professor V. BOONPUCKNAVIG, Bangkok/Thailand. The section on neoplasias contains the special chapter "Kaposi's Sarcoma" by Professor H. ITAKURA (Department ofPathology, Institute of Tropical Medicine, Nagasaki University, Japan). We have not included a special chapter on HIV-related diseases because the problem of HIV is no longer specific to the tropics; how ever, other chapters contain corresponding references to special questions relating to HIV. This volume analyses the tropical diseases in 26 chapters written by 29 con tributors from 10 countries (Australia, Brazil, Germany, Japan, Malaysia, South Africa, Sultanate of Oman, Thailand, United Kingdom and USA). It is a con centrated, current review of tropical pathology also including some aspects of epidemiology and clinical features. Concerning the wealth of topics it was inevitable that overlappings occurred between microbiological or epidemiological aspects and organ-specific appro aches. For example, Syphilis is one important topic of spirochaetal diseases (Chapter 7) and also a sexually transmitted infection (Chapter 10). Therefore, some recapitulations are a concession to this aspect. Unfortunately, one planned chapter ("Tropical Neuropathology") could not be integrated in this volume. In spite of the binding acceptance of the designated author to take over this chapter the author was not in the position to realize the submission of the chapter. But to avoid a further delay of the publication the volume was printed without this chapter. We request the readers to apologize our decision. The editors would like to thank the authors, all reputed experts in the various fields of tropical pathology, very much for their hard work and cooperation. We sincerely hope the readers of the volume will benefit from our efforts.
