It is an exciting and practical field in which to become involved, but developing skills, competence and expertise to undertake endoscopy can be technically demanding. This is even more true when the endoscopist begins to undertake therapeutic procedures. The skills required can only be acquired by practice and application. The trainee rightly has to focus on these skills and there are several `how-to-do-it' books to help. In clinical practice, the questions of 'why do it, where to do it, and when to do it (or not!)' are often more pressing and we hope to address these questions in this book, in addition to practical advice on technique. The book therefore should be useful to endoscopists during the course of training but our co- authors have also included challenging areas which will interest even the established endoscopist. We have also included two chapters on screening as this ought not to be undertaken unless realistic therapeutic options are intended. We are profoundly grateful to our friends who have contributed to this book. They bring a wealth of personal experience in the field, which has been acquired over many years. We hope that the concentration of that experience into this text will help you to make the right decisions for your patients. Inevitably, in a multiauthor tome, there will be repetition. We took editorial licence in moving some text between chapters among different authors for the purpose of integrating the material.
