It is a unique product in many ways, not just another textbook. It may be the only allergy text produced by clinicians (and a few basic scientists) from the West (mainly the US) and the East (mainly India). Indeed, the U.S. is represented (among other states) by authors from 10 states and India is represented by contributors from Vellore, Mysore, Bangalore and Pune. These differing origins make for interesting perspectives. The text is designed to explicate the practice of allergy, both pediatric and adult. There is indeed some basic immunology but the emphasis is on the allergic patient—her (or his) history, physical examination, laboratory evaluation, environment and treatment, including specifi c immunotherapy as well as pharmacotherapy and environmental control. Thus the “target audience” for this effort includes medical students, physicians, nurses and paraprofessionals who interact with patients who do have or might have illnesses with allergic components and who need expert handling. Of particular interest with regard to the international fl avor of this text are chapters on: Food Allergy Environmental factors such as air pollution Allergy Skin tests Allergy practice: East vs. West Aerobiology Allergen Immunotherapy Controversies in Allergy Alternative treatment techniques Personalized medicine Occupational asthma Collaborative and intersectional teaching efforts present challenges for the authors and editors and in this case the fi nal product is a valuable addition to the literature on allergic diseases. Henry N. Claman, M.D. Distinguished Professor of Medicine University of Colorado School of Medicine Anschutz Medical Campus Aurora, Colorado USA Preface This endeavour to put together an international textbook of allergy/immunology began over two years ago. The concept of who this book would serve and its contents thereof gradually crystallized over the ensuing years and the fi nal result is the ‘Textbook of Allergy for the Clinician’. The chapters cover the core topics in the Allergy-Immunology discipline and are written by truly an international set of authors from America, Asia and Europe. This is the fi rst textbook in the fi eld of allergy, asthma and immunology covering such varied material with chapters written by a highly qualifi ed team of authors with diverse expertise. This is also a very timely textbook, since for the fi rst time, there is growing realization of combining evidence based data from ‘East and West’ in our professional understanding, rather than in two separate compartments. The infl uence of eastern thought is complimented by the ‘scientifi c’ approach of the West in understanding and formulating our approach to medicine. There are 26 authors of eastern origin and 25 authors of western origin who have come together to create this book. The book contains 33 different chapters which, in addition to the basic allergy and immunology subjects, contain chapters on: Personalized medicine and asthma, the effects of air pollution on allergies, Aerobiology with color pictures, environmental issues, Allergy practice: east vs. west, Yoga in asthma, Ayurveda and herbal medicines. Since this book deals with not only basics, but also the most recent thoughts on clinical approach and management, it should appeal to a wide audience, from medical students to residents, fellows in training as well as specialists. All the authors have been extremely graceful in their submission of their respective chapters. Dr. Henry Claman, our mentor and guide, has summarized the concept of this textbook very well in his foreword and we are extremely appreciative of his words of wisdom and vision. Our families have been more than cooperative; they have sacrifi ced on several occasions to allow us to complete this textbook. Our sincere thanks of appreciation to International Asthma services (IAS), Denver, Colorado USA and Dr Lokeshwararao Edara, Battle creek, Michigan USA for fi nancial support. August 2013 Dr. Pudupakkam K. Vedanthan Dr. Harold Nelson Dr. Shripad Agashe Dr. P.A. Mahesh Dr. Rohit Katial Acknowledgements Pudupakkam K. Vedanthan I dedicate all my work and efforts in this endeavor to my beloved teachers and my family. I feel fortunate and blessed to have been offered the opportunity of serving as Lead Editor for the Textbook of Allergy for the Clinician. In my 38 years of practicing Allergy, Asthma and Immunology in USA and several developing nations around the globe, I am convinced that such a task is an essential part of educational outreach. I was fortunate to have had stalwarts in the specialty to be my mentors, namely Dr. Elliott Middleton (deceased), Dr. Hyman Chai (deceased), Dr. Harold Nelson and Dr. Henry Claman. These great personalities have enabled me to develop the vision, strength and values in my pursuit of professional excellence. This textbook has involved signifi cant sacrifi ces of personal time and comforts over the past three years by my family, especially by my wife, Kamala, who has been very supportive and encouraging. My daughter, Ranjani, has helped me immensely in editing several chapters burning the midnight oil. The constant words of encouragement and support from my son, Dr. Rajesh Vedanthan was very welcome. I also would like to dedicate this work to my deceased father Dr. P.D. Krishnasawmi, a popular medical practitioner in Mysore, India and my deceased mother Mrs. Seethammal, for their upbringing and instillation of high ideals. The publishers, CRC Press have been extremely gracious to support and guide me through this whole process. I am highly appreciative of the excellent cooperation of my co-editors, in particular Dr. Harold Nelson, who guided me and helped me meet the deadlines. On behalf of the editorial board, I thank all the authors and co-authors for their tremendous work, for submitting their chapters in a timely fashion, as well as for being patient and cooperative through the multiple steps involved in presenting this work to you all. Shripad N. Agashe I wish to dedicate the chapter on Aerobiology to my late wife Dr.Nirmala S.Agashe, retired Professor of Mathematics, Bangalore University who sacrifi ced so much and encouraged me to continue my academic pursuit. I am also thankful to my daughters Sandhya and Swapna who looked after me all these years, helped me and encouraged in all my academic work. Dr. P.K. Vedanthan did the lion’s share of the book preparation work, for which I am grateful.
