TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL SECTION 02130 HAZARDOUS MATERIALS ABATEMENT AND CONTROL PART 1 - GENERAL 1.1 SUMMARY A. Section includes requirements and procedures to be performed by the Contractor for the handling, removal, and disposal of hazardous materials including but not limited to asbestos-containing materials, lead based paints, and demolition associated with hazardous materials. B. Hazardous materials removal shall be conducted as per the construction phasing and staging described as specified in the drawings. C. Standard 90A: “Fire Rating of Sprayed-On Fireproofing.” 1.4 DEFINITIONS A. Activity Class/Category - Lead: Lead hazard designations assigned to work activities that involve lead-containing materials. Activities, which fall into Classes I through III, including as examples the operations defined below, are required to assume the following personal airborne exposure levels, unless otherwise demonstrated.  Surface clean-up of lead-containing dust or debris less than 15,000 microgm/ft2  Spray painting with lead-based paints Activity Class I Exposure  Manual demolition of structures (e.g. drywall, plaster, etc.) Less than 500 microgram/m3  Manual sanding, grinding, needle gunning, chiseling, hammering, wire brushing, milling or scraping of lead-based coatings  Head gun removal of surface coating power tool Power tool cleaning with dust collection system  Using lead mortar  Lead burning Activity Class II Exposure500  Rivet busting microgram/m3 or greater but less than 2500 microgram/m3  Power tool cleaning without dust collection systems  Clean-up of dry abrasive  Abrasive blasting enclosure movement and removal  Abrasive blasting of coated surfaces Activity Class III exposure  Welding on coated surfaces 2,500 micrograms /m3 or greater  Torching or cutting of coated surfaces  Torch burning of coated surfaces Contract No. 2762R Port Of San Francisco 02130-2 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL B. Asbestos Work Class: Activities for removing asbestos materials by categories as follows:  Activity involving removal of Thermal System Insulation (TSI) Work Class I and surfacing asbestos-containing materials (ACM) or friable presumed –asbestos containing materials (PACM)  Activity involving removal of ACM which is not TSI or surfacing material, including, but not limited to, wallboard, Work Class II floor tiles and sheeting, roofing and siding shingles, and construction mastics.  Repair and maintenance operations where TSI or surfacing is Work Class III likely to be disturbed, which fits within one standard glovebag or waste  Maintenance and custodial activities during which employees contact but do not disturb PACM or ACM and activities to Work Class IV clean up dust, waste and debris resulting from Work Class I, II, and III activities 1.5 SUBMITTALS A. As per Section 01300-Submittals and Section 01350- Hazardous Materials Procedures, the Contractor shall submit a Hazardous Materials Management Plan (HMMP with the following documentation listed below. The HMMP shall be submitted within (10) ten days after the Notice to Proceed and before commencement of demolition activities. No hazardous materials work will start without the HMMP approved and reviewed by the City and its designated industrial hygiene consultant. B. The Hazardous Materials Management Plan (HMMP) is the Contractor’s comprehensive plan for the management of hazards encountered during the Work of this project. The HMMP is to include the following: 1. Information about the Contractor’s designated Site Safety Representative as per Part 1.06 of this Section and Section 01045 - Health and Safety Criteria. Include his/her training certification, qualifications; his/her name, phone number; fax number, and pager number. 2. Management spill procedures in the event of asbestos or any hazardous materials release or any event that may require modification or abridgment of site control and decontamination procedures 3. Intended methods of compliance for hazardous materials handling work, including description of engineering controls, personal protective equipment as well as compliance monitoring as applicable. 4. Schedule and sequence of work for all hazardous materials work. Contract No. 2762R Port Of San Francisco 02130-3 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL 5. A copy of the Site-Specific Hazard Communication Plan in accordance with Federal and California OSHA requirements. 6. Copies of required licenses, certifications and notifications to handle and control hazardous materials as applicable 7. Name, address and phone number of the Contractor’s landfill as applicable. C. For Asbestos Containing Materials (ACM), as part of the Hazardous Materials Management Plan (HMMP) the Contractor shall submit the following but not limited to; 1. Asbestos Pre-job Submittals: a) Proof of current asbestos contractor's license issued by the California Contractors’ State License Board. b) Proof of current California Department of Health Services (DOHS) Asbestos Contractor's registration certification. c) Valid and current Bay Area Air Quality Management District (BAAQMD) notification for the Project (as applicable). d) Cal/OSHA 24-hour notice per 8 CCR 1529. e) Worker documentation, including: 1) Current AHERA training certifications supervisor/competent person 2) Current AHERA training certifications for workers. 3) Respiratory fit test records within past 6 months. 4) Annual medical examination approvals for respirator use. f) Written asbestos abatement work plan and schedule with the sequence of work. g) Material Safety Data Sheets (MSDS) for chemicals used. h) Emergency phone number and pager listing. i) DOP testing of negative pressure units and vacuums. (on-site test) j) Rotameter calibration data within past 6 months. 2. Periodic Submittals: Submitted upon request during abatement: a. Contractor’s personal air monitoring results (daily) b. Updated workers documentation (as needed) c. Daily boundary access logs, as applicable d. Daily negative pressure records, as applicable e. Copies of updated schedules and notices to the regulatory agencies (as- needed.) 3. Project Closeout Submittals: Within 5 calendar days upon the City’s request or within 5 calendar days after completion of the abatement or hazard control work, the Contractor shall submit the following: a. Copies of updated schedules and notices to regulatory agencies, as needed. Contract No. 2762R Port Of San Francisco 02130-4 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL b. Receipt and weight tickets from landfill operator or incinerator, as applicable. c. Copies of completed uniform waste manifests. d. Certification of Completion. D. For Lead Based Paint 1. Pre-job Submittals: As part of the Contractor’s Hazardous Materials Management Plan (HMMP) the Contractor shall submit the following but not limited to: a. Worker documentation: 1) Current DOHS lead contractor/supervisor training certificate. 2) Current lead awareness training certificates - workers or DOHS Certified Lead Workers Certificate, as appropriate. 3) Respiratory fit test records within past 6 months. 4) Annual Medical Examination approvals. 5) Blood lead tests within past 90 days. b. Material Safety Data Sheets for chemicals used. c. Lead Hazard Control Plan pursuant to 8 CCR 1532.1: Procedures for minimizing and controlling the migration of lead from disturbance of lead- containing materials including a written lead hazard or lead abatement work plan and schedule with the sequence of work. d. For exterior lead base paint removal, submit a Copy of the “Notification of Paint Disturbing Work” of the San Francisco Building Code Chapter 36, Section 3604.3 whenever this notification is required. e. For steel structures coating hazards associated with any removal, disturbance, and preparation of any and all paints during the contract work the following shall be submitted: (1) A copy of the current SSPC QP1 and QP2 certification (2) Containment and Ventilation System as required (3) Waste Collection and Disposal Plan 2. Project Close-out Submittals: Within 5 calendar days upon the City’s request or within 5 calendar days after completion of the abatement or hazard control work, the Contractor shall submit the following: a. Updated worker documentation, as needed. b. Contractor periodic personal air monitoring results. c. Receipt and weight tickets from landfill operator or recycler as applicable. d. Waste profiling data (TCLP, WET, and SW846, as applicable). Contract No. 2762R Port Of San Francisco 02130-5 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL E. For PCB Ballast: 1. As part of the Hazardous Materials Management Plan (HMMP) the Contractor shall submit the following but not limited to: a. Hazard Control Plan: Procedures for clean-up of leaking ballasts and disposal and transportation for incineration of PCB ballasts b. Evidence of hazard awareness training of workers removing and packing PCB ballasts. c. Identification of EPA approved incinerator and DOT approved transporter. d. PPE to be used. 2. Project close-out Submittals: Within 5 calendar days upon the City’s request or within 5 calendar days after completion of the abatement or hazard control work, the Contractor shall submit the following: a. Completed Uniform Waste Manifest. F. For Fluorescent Light Tube: 1. As part of the Hazardous Materials Management Plan (HMMP) the Contractor shall submit the following but not limited to: a. Identification of EPA approved recycler. b. Temporary storage plan. 2. Project Close-out submittals: Within 5 calendar days upon the City’s request or within 5 calendar days after completion of the abatement or hazard control work, the Contractor shall submit the completed manifest or evidence of shipment date, recycler and quantities shipped. G. For Copper Chromate Arsenate (CCA) Treated Wood: 1. As part of the Hazardous Materials Management Plan (HMMP) the Contractor shall submit the following but not limited to: a. Identification of EPA approved hazardous waste landfill disposal or an EPA approved solid waste disposal facility b. Temporary storage plan. 2. Workers Documentation: a. Certification of the workers and supervisor’s forty (40) hour HazWoper training in compliance with 40 CFR 1910.120. Contract No. 2762R Port Of San Francisco 02130-6 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL b. Medical examination approvals for respirator use within the past twelve (12) months, or in compliance with 8 CCR 5144. c. Respiratory fit test records within the past twelve (12) months minimum, or in compliance with 8 CCR 5144. 3. Within 5 calendar days upon the City’s request or within 5 calendar days after completion of the abatement or hazard control work, the Contractor shall submit the completed manifest or evidence of shipment date, recycler and quantities shipped. H. For Mold Remediation 1. As part of the Hazardous Materials Management Plan (HMMP) the Contractor shall submit the following but not limited to: a. Mold Remediation Plan, including use of personal protective equipment, containment construction, entry and egress procedures, hygiene requirements, and waste disposal procedures, appropriate for the level of work described herein. b. Material Safety Data Sheet for each chemical product used and brought onto the worksite. 2. Workers Documentation: a. Certification of the worker’s awareness or hazards training in accordance with the provisions of the Hazard Communication Standard, 8 CCR§5194 by a Certified trainer or as stated on the Contractor’s letterhead by the Contractor’s Health & Safety Officer or Superintendent. Individuals involved in project work and potentially exposed to mold- contaminated materials are to be trained in the health hazards associated with mold exposure and the steps the contractor will take to minimize those hazards. b. Medical examination and approval for use of respiratory protection, as applicable, including current respirator fit test records. 1.6 SITE CONDITIONS A. Where the analytical data contained in the hazardous materials survey reports, as disclosed in Section 00300- Reference Documents does not clearly characterize the following conditions or materials, the Contractor shall assume, proper management and control when working around the following assumed hazardous building materials: 1. Thermal systems insulation. 2. Sheetrock and plaster walls and ceilings. Contract No. 2762R Port Of San Francisco 02130-7 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL 3. Acoustical ceiling plaster in the hallways. 4. Structural fireproofing. 5. Laid in ceiling tiles. 6. Wall heater transite behind radiators 7. Transite pipes, flue panels 8. Wall vinyl composition sheets 9. Vapor barriers underneath floors. 10. Wiring in electrical equipment. 11. Fire doors. 12. Vinyl sheet flooring and underlying mastic 13. Vinyl floor tiles and underlying mastic 14. Insulation board behind heaters 15 Roofing materials 16 Arsenic Treated Wood (CCA) 17. Mold and Fungi B. Painted or coated surfaces at the Work site are known or are suspected to contain lead. The City has not verified that any paints coatings, dusts, ceramic tile or other materials are “lead free.” The following materials are assumed to contain lead and other hazardous material unless proven otherwise: 1. Glazed ceramic tiles 2. Paint on stucco (plaster, wood) walls 3. Paint on wood trims 4. Paint on asphalt floor s 5. Paints on metal surfaces and structures C. Unexposed or uncharacterized surfaces have the potential to create a health and environmental hazard and the control of this condition is part of the Work of this Contract: 1. Transite pipelines/panels 2. Gaskets Contract No. 2762R Port Of San Francisco 02130-8 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL 3. Structural steel, when painted, has that paint present between bolted, riveted, or other connections and the direct or indirect disturbance of that connection poses a health and environmental hazard. 1.7. QUALITY CONTROL A. MEETINGS 1. Pre-Abatement Meeting: Upon City’s approval for abatement, a pre-abatement meeting will be conducted at the City’s discretion. The Contractor is to attend a pre-construction meeting with the City representatives, the City’s Consultants, and other Subcontractors whose work may be affected. The meeting agenda shall include the following considerations: a. Review of the Specifications and Plans in detail related to the abatement and hazards work. All conflicts and ambiguities, if any, shall be discussed. b. Review in detail the project conditions, schedule, construction sequencing, abatement application requirements, and quality of completed work. c. Review in detail the means of protecting adjoining areas; protect of Contractor’s, Subcontractor’s, City’s workers, and completed work during the abatement activities. d. Pre-job submittals requirements. e. Site security requirements. B. FIELD QUALITY CONTROL SAMPLING 1. Perimeter Lead Air Monitoring: The City and it’s Consultants may elect to perform air sampling in non-work areas of the building before the start of demolition and/or abatement work to establish the background total lead concentration and/or conduct monitoring during lead hazard-related work such as demolition, refinishing, or torching and welding activities, to review containment effectiveness and acceptability of the Contractor's work practices. The City at its own discretion may collect air samples for analysis by flame atomic absorption a. Air samples will be analyzed for total lead in accordance with NIOSH method 7082. The perimeter background concentrations outside the cordoned or contained abatement area(s) shall not exceed an airborne concentration above OSHA's "Action Level" of 30 micrograms per cubic meter (30 µg/m3), averaged over an 8-hour period, for all samples. b. Air sampling results in excess of the Cal/OSHA “Project Action Level” of 30 micrograms per cubic meter (µg/m3) within the construction zone may require isolation of the work area, upgrades in the required respiratory protection, amendment of work procedures, and/or clean-up of the affected area. Contract No. 2762R Port Of San Francisco 02130-9 TECHNICAL SPECIFICATIONS DIVISION 2: HAZARDOUS MATERIALS ABATEMENT AND CONTROL c. Air sampling results in excess of the EPA's National Ambient Air Quality Standard (NAAQS) of 1.5 µg/m3 at the site's property line or at adjoining occupied non-construction areas may require isolation of the work area, amendment of work procedures, and clean-up of the affected area. d. Re-sampling of the contaminated areas and handling, shipping, and analysis charges (including the City or it’s Consultant’s time and expenses) for additional sampling required to show background levels below these lead standards shall be borne by the Contractor. 2. Perimeter Lead Wipe Samples: Wipe samples may be collected outside or within representative work areas prior to start-up of the abatement activities in order to establish the background or pre-existing lead dust content or at the option of the City when evidence of contamination of Non-Abatement Areas is visually apparent, a. The City may evaluate the lead dust concentrations outside the work area on the walls and floor during the work progress by collecting wipe samples to evaluate the integrity of the containment and to detect dust contamination resulting from: 1) Failure to adequately cordon off or contain work area dusts, clean-up debris, and use approved work practices, such as wet wiping and HEPA vacuuming; 2) Failure or breaches in the work area isolation containment, as applicable; 3) Failure or rupture in the negative pressurization/HEPA filtration system, as applicable; 4) Incomplete decontamination of personnel or equipment removed from the work area(s). 3. If the test results indicate that any of the samples is greater than the background concentrations or 400 µg/ft2, whichever is greater, the Contractor shall: a. Immediately and automatically stop all work, until corrective actions are completed. b. If the sample was collected inside the building but outside the critical barriers or within the decontamination assemblies, isolate and decontaminate the affected area using a HEPA-vacuum and wet cleaning methods.
