So, the reason these look the same is because this particular nurse starts all of her visit notes the same way. And unfortunately, the space given doesn’t allow for all of the other information to be included. Here are the full notes from each visit: 12/15: Routine visit with client. Invited into clean nicely furnished home Home is cool and all utilities are on. Present at visit are Mom and infants Mom assessment completed and documented. VSS. Denies any medical concerns at this time. All education completed and documented. Discussed nutrition and what to feed baby. Discussed support persons important to family. client is bottle feeding. Client is under a lot of stress has anxiety currently taking xanax that is prescribed. States she got her meds stolen from her Infant assessment completed and documented. VSS. Skin warm and dry. Lungs clear with good aeration and respiration even and unlabored. Infant is clean and dressed appropriately for weather. Physical assessment WNL. Weight gain and development appropriate for age. Education and anticipatory guidance completed and documented. Growing well and eating good. talked with client about tummy time and not leaving the alone because they are starting to roll. discussed ways to sooth teething also . Plan is to revisit per NFN policy. Mom encouraged to call with any questions or concerns prior to that visit. Mom verbalized understanding of all information and agreeable to plan.
