It is this human drive that compels many of us to go into healthcare and serve our communities. Unfortunately, our ability to provide access to cost-effective care has diminished every year for the past three decades. This is a direct result of our inability to change quickly in an era of high competition and lower reimburse- ments. My motivation in writing this book was based on the sincere desire to advance the evolution of healthcare through best practices for all communities. At 18% of GDP, the US healthcare system is, without rival, the most expensive in the world. For three decades, our great- est leaders have been working to improve cost, quality, and service, without much success. Although many organizations have learned to do well, some still do not know the basic principles that lead to high performance. As pressures from regulatory agencies, consumers, and competition heat up in the next decade, many organizations will find themselves des- perate for practical solutions for their very survival. This book is a culmination of my 25 years of healthcare Performance Improvement (PI) experience in top-performing systems across the United States, and my attempt to dissemi- nate operational best practices by sharing what has worked and what has not. It is a step-by-step “how to” book to help set up basic programs around traditional expense reduction ix
