T he concept of this book was conceived many years ago when Barend Haeseker
was one of my assistants during his training in plastic surgery. He is perhaps the
world’s leading historian and a prolifi c writer in Dutch on the subject of plastic
surgery, and it is to his enthusiasm, knowledge and help with this collection of
vignettes that I am greatly indebted.
I am very grateful to Penelope Hopwood for typing and retyping the text as well
as crossing swords with me on the grammar and punctuation.
