NEWYORK-PRESBYTERIAN HOSPITAL ABBREVIATION LIST Revised: November 1999 INTRODUCTION The following is the revised Hospital Abbreviation List approved by the Executive Health Information Management Committee in November 1999. This revision is based on input from Joint Forms Sub-committee Members, various Hospital Departments, and practitioners reflecting standard nomenclature and practice at NewYork-Presbyterian Hospital. An abbreviation (or symbol) may be used in the medical record only if on the approved Abbreviation List, or on a legend containing the abbreviation on the form intended for use. All forms used in the medical record must be approved by the Joint Forms Subcommittee. The Abbreviation List contains three sections for cross referencing: (1) Alphabetic Sorting by Definition; (2) Alphabetic Sorting by Abbreviation; (3) Computer System Abbreviations.
