JOINT SECRETARY: Dr Rungmei SK Marak Antibiotics have transformed the practice of medicine, making once lethal SGPGI Lucknow infections readily treatable and making other medical advances, like cancer TREASURER: chemotherapy and organ transplants, possible. But on the other hand with Dr Vineeta Mittal indiscriminate and improper use, the Multi drug resistance has emerged bringing us RMLIMS Lucknow again near to status similar to Preantibiotic era. We are fighters and for survival of f ittest we are again adopting and developing new strategies to control and combat this menace. Antibiotic stewardship and Biomedical waste management are amongst Executive council one of these strategies . I extend my sincere thanks to all the members of the Association for bestowing faith in me. I am also grateful to all my predecessors and founder members for their hard work for bringing the Chapter to this level. We still have to share great responsibilities to let it blossom to its full strength. I have seen vast improvements and great changes in Microbiology during the last 33 years of my academic career. Information technology, molecular biology & automation have made great inroads in our speciality. Our commitment is to provide continuous updates to the younger generation in the field of all the subspecialties of Microbiology and we were successful to a large extent by organizing a grand annual event “UP-UK Microcon-2017” in Dehradun in February 2017, and the organizers deserve appreciation. I have felt that no free oral session is held during our state chapter conferences. Since, oral presentation helps in improving the communication skills for our post-graduates (PGs), we are therefore, going to devote time for such session for our PGs only. Moreover, there was demand for a separate best paper award session for MD students. The executive committee will examine the issue and place its recommendation before the general body. Hopefully, the first circular for “UP-UK MICROCON-2018” will come out shortly. As clinical microbiologists, we have lot of responsibilities to share with the clinical counterparts, especially in the areas of diagnostics, infection control and antibiotic stewardship programme. It is high time to introduce newer techniques to reduce the turnaround time without compromising the quality. There is a need to disseminate practical knowledge through organizing workshops and introducing hands-on-training programmes by the centres of excellence. I am confident that with the support of all of you, we shall be able to take our specialty to a new height. With best wishes Prof K.N.Prasad Professor ,Department of Microbiology SGPGI, Lucknow President IAMM UP UK Chapter( 2017-18) NEWS LETTER IAMM-UP UK CHAPTER ( APRIL 2017) Volume 4 Issue 1 FROM SECRETORY DESK SECRETARY DESK Greetings to all the members of IAMM-UP & UK Chapter. Anil Kumar MD Topic : The top 10 things Microbiologists wish, every clinician knew. 1 ) A CME was held on 25 th March 2017, at Subharti Medical College on an interesting topic, “ The Top 10 Things Microbiologists Wish, Every Clinician knew.” By Dr V. Anil Kumar It was well attended by about 100 post graduate students and 50 faculty members of different clinical departments of Subharti Medical College. There was very useful interactive interaction between the speaker and the clinicians of the college, after the presentation. Topics discussed included: clinical significance of skin commensal in blood culture, value of follow-up blood culture, fungemia and sepsis, futility of regular anaerobic blood culture, and clinicians ignorance about inherent drug resistance among common bacterial blood isolates. 2) After the CME Dr V. Anil Kumar also gave a talk about “Basics of blood culture” in the department of Microbiology. Apart from 8 faculty member and the post graduate students; 7 pursuing MD(Micro.), 12 MSc (Medical Micro.) and 13 PhD (Medical Micro.) also attended and interacted with Dr Anil, in the department of Microbiology, SMC, Meerut. His presentation covered why and when to advice blood culture and how to go about it for better patient care. He discussed pre-analytical, analytical and post analytical phases and precautions to be taken during each phase. The Dos and DONTs during collection and transport as well as phrasing of reports and comments to follow different reports were also discussed at length, for the benefit of postgraduates. \ DR ANIL, DELIVERING LECTURE IN CME, IN COUNCIL HALL OF SUBHARTI MEDICAL COLLEGE NEWS LETTER IAMM-UP UK CHAPTER ( APRIL 2017) Volume 4 Issue 1 ELIZABETHKI NGIA -AN EMERGING PATHOGEN Author- DR BALRAM JI OMAR (AIIMS,RISHIKESH) DR UPASANA BHUMBLA (GEETANJALI MC,UDAIPUR) HISTORICAL INTRODUCTION Elizabethkingia meningoseptica is a gram-negative rod-shaped bacterium widely distributed in nature (e.g. fresh water, salt water, or soil). It may be normally present in fish and frogs but is not normally present in human microflora. In 1959, the American bacteriologist Elizabeth (who isolated Kingella kingae in 1960) was studying unclassified bacteria associated with Pediatric meningitis at the Centers for Disease Control and Prevention in Atlanta, when she isolated an organism (CDC group IIa) that she named Flavobacterium meningosepticum (Flavobacterium means "the yellow bacillus" in Latin; meningosepticum likewise means "associated with meningitis and sepsis"). In 1994, it was reclassified in the genus Chryseobacterium and renamed Chryseobacterium meningosepticum (chryseos = "golden" in Greek, so Chryseobacterium means a golden/yellow rod similar to Flavobacterium). I n 2005, a 16S rRNA phylogenetic tree of Chryseobacterium showed that C. meningosepticum along with C. miricola (which was reported to have been isolated from Russian space station Mir in 2001 and placed in the genus Chryseobacterium in 2003 were close to each other but outside the tree of the rest of the Chryseobacterium and were then placed in a new genus Elizabethkingia named after the original discoverer of F. meningosepticum.  It is an emerging nosocomial pathogen among immunocompromised patients. It is a gram negative rod, non-fermenter, non-motile, oxidase positive and is widely distributed in nature, particularly in soil and water.1-2  The organism has tendency to survive at places apart from water supplies, sinks, taps, saline solutions, disinfectants. It can also survive in chlorine treated municipal water supplies, often colonising sink basins, intubation tubes, humidifiers, incubators of new borns, where it has become a competent threat for infections in the hospital set up.3-5  Elizabethkingia meningoseptica is multidrug resistant organism including extended spectrum beta lactams and aminoglycosides, acting as a serious challenge for the clinicians to treat. Elizabethkingia spp. possess two different types of beta lactamases, namely class A extended spectrum beta lactamases (ESBLs) and class B metallo beta lactamases (MBLs); the latter confers resistance to carbapenems, which are widely used to treat infections caused by multidrug resistant gram negative bacteria. 4
