14:3:1:3:3:1:4 Vaccine Series: receipt of all doses listed above 4 doses of DTaP, 3 doses of Polio, 1 dose of MMR, 3 doses of Hib*, 3 doses of Hepatitis B, 1 dose of Varicella and 4 doses of Pneumococcal
*Full series Hib: ≥3 or ≥4 doses of Hib vaccine depending on product type received (includes primary series plus the booster dose).
**Rotavirus: ≥2 or ≥3 doses of Rotavirus vaccine, depending on product type received (≥2 doses for Rotarix® [RVI] or ≥3 doses for RotaTeq® [RV5]).

The National Immunization Survey (NIS) is performed by the Centers for Disease Control and Prevention (CDC) and
collects statistics for each state on the immunization coverage for children 19 through 35 months of age between
January and December every year. Statistics are released every September for the previous year. For information on
the NIS methodology and previous years, visit http://www.cdc.gov/vaccines/stats-surv/nis/default.htm.

NIS vaccine coverage definitions:
• 4+ DTaP: 4 or more doses of diphtheria, tetanus, and pertussis vaccine
• 3+ Polio: 3 or more doses of poliovirus vaccine
• 1+ MMR: 1 or more doses of measles, mumps, and rubella vaccine
• 3+Hib: 3+ or 4+ doses of Haemophilus influenzae type b vaccine, depending on product type received
• 3+ Hep B: 3 or more doses of hepatitis B vaccine
• 1+ Varicella (chickenpox): 1 or more doses of varicella vaccine
• 4+ PCV: 4 or more doses of pneumococcal conjugate vaccine
• 2+ Hep A: 2 or more doses of hepatitis A vaccine
• 2+ Rotavirus: 2 or more doses of rotavirus vaccine, depending on product type received
