The book was well received but with the remarkable development of new imaging modalities producing images of ever-improving quality, it was agreed that a new edition of the book should refl ect this and also be rather more interactive. Tragically, Peter died much too soon, but this volume has been co-edited with two outstanding colleagues, Philip G. Conaghan and Philip J. O’Connor. Together, we have attempted to pro- duce an exciting, readable and well-informed volume that would be a genuine advance on the original volume. We would like to thank our con- tributors for (mostly!) keeping to the tight publishing schedule and hope that the reader will enjoy the breadth of topics covered, the quality of the images and the detailed information provided.
