Salah Mansour Preface In recent years, most studies on ear-related illnesses and their treatment have focused on the latest advances in implants, especially cochlear implants, with their particular technical and rehabilitation requirements. While positive in one sense, this situ- ation has unfortunately negatively impacted the cur- rent body of available resources and innovative studies on middle ear pathologies; specifically for the purposes of teaching medical candidates and other academic and clinical users in Otology. This book has been produced to shed light on the latest advances that have been achieved in relevant complementary disciplines addressing the middle ear, such as innovations in the understanding of its functional anatomy and breakthroughs in physiopathology which candidates are rarely exposed to. In addition, new audiological investigative methods and advanced technical approaches in imaging which have had formidable impacts in the diagnosis and therapeutic processes of these pathologies have also now been made available. In preparing this book, we have drawn on many sources that seemed to have advanced applicable merit and we reported these sources accu- rately in the references. As a result, a rich and diverse evidentiary base has allowed us to build a new multidimensional methodology in the diagnosis of middle ear ailments and their treatment. As such, this book is equally timely for candidates, academics, and specialists as it represents a valuable up-to-date learning resource, based on demonstrated clinico- radiological correlations. It also gathers, under one single cover, most illnesses that relate to the middle ear and their treatments. The reader will find this book to be a valuable tool when faced with a difficult diagnosis and when needing to determine next steps before a complex pathology of the middle ear, especially if contemplating proceeding with surgery. vii viii Preface In closing I want to thank warmly my colleagues, the co-authors, for their devo- tion and dedication in working closely together to end up with such achievement. My special thanks go to my friend Prof Jacques Magnan who, with his unique expertise, his wide experience and huge knowledge, gave to this work an invaluable contribution. Beirut, Lebanon Salah Mansour Preface ix Over the past two decades, progress in otology largely derived from technological improvements, rather than the accomplishments of clinicians and pathologists. Among such improvements imaging technology has dominated the field, sometimes to the detriment of clinical observations, with some practitioners forgetting that radiological exams are intended to complement clinical work. Indeed, radi- ologists were trained to look to the clinician’s inputs in order to guide their radiological analyses and draw diagnostic conclusions. This evolution highlights the increasing value of correlations drawn from combined clinical and radiological efforts, as strategic vec- tors that guide therapeutic attitudes and otological management. It is in light of this new reality that Professor Salah Mansour called us back to work in order to assemble and harmonize (conveniently into a single book) the achievements of basic sciences, together with the clinical, pathological and radio- logical data that lead to enlightened diagnostics and surgical decisions. Our ambi- tion is to provide an update on recent developments regarding the diseases of the middle ear, and outline the space within which the rules of surgical otology will continue to evolve. We hope that talented individuals will benefit from such outline and contribute to the progress of our practice. Colombiers, France Jacques Magnan x Preface Nowadays the close correlation between clinical evaluation and radiological observation is the key to successful diagnostic and follow up inquiries regarding most middle ear diseases. By combining advanced anatomical knowledge with clinical and scannographic findings, radiolo- gists were able to develop useful algorithms that placed modern imaging techniques into the stream of disease management. My commitment to oto- radiology during the past 15 years has allowed me to tackle the challenge of illustrating most patho- logic entities, and I strongly believe that excellent and comprehensive imaging has an important impact in otology practice. Working on this book was a unique opportunity for me to learn from the great experience of my respectful co-authors, and I am proud to have contributed to this great work. I have learned a great deal from them, and owe my profes- sional success to their teaching and mentorship. This book would not exist as it is without the great contribution of Dr. Karen Nicolas, a brilliant otora- diologist who is expert in the field of temporal bone imaging. This book is the happy culmination of our keen and long-drawn focus on middle ear diseases. It addresses complex facts and controversial issues in the field. Lucid presentation was of particular importance when approaching each subject, as we intended to facilitate the easy recall and reproduction of concepts by students and practitioners. We intended this book for our departments, residents and fellows, our colleagues, and the patients who will ultimately benefit from advances in our field.
