The Congress in Toulouse, MIE-81, from 9 - 13 March 1981, is the third congress in the ser ies following MIE-78 in Cambr idge, and MIB-79 in Berlin with a gap during 1980 for the world congress MEDINFO-80 in Tokyo. The rationale behind all these congresses is the scientific need to share results and ideas and the educational need to train a wide variety of professional staff in the potential of health care and medical informatics. All the caring professions are involved, doctors, scientists, nurses, para-medical staff, administrators, health care planners, community physicians, epidemiologists, statisticians, operations analysts together with specialists from the computing profession dealing with system analysis, hardware, software, languages, data-bases and the marketing of systems. Medical Informatics is a very wide subject with ramifications throughout the health care and preventive services; it offers a key to the monitoring and improvement of patient care and to the provision of a healthier environment. The collection and evaluation of relevant data improves our understanding of the ways in which health care is provided while the availability of cheaper computer hardware and more versatile software enables us to design and implement more revealing and intelligent medical systems. Even though typical systems take a substantial amount of time to design, implement and evaluate, there is the continuing need for informaticians to assess the current state of developmen.t across the whole field with their colleagues and for new-comers to the profession to be exposed to the variety of systems in current use and the extent of the role that medical informatics can play in health care. The rapid pUblication of conference papers from a multi-stream congress enables participants to follow work presented to sessions that they could not attend but more importantly this approach provides a published record with a relevant bibliography for research workers to asse~s which groups are active in which areas of medical informatics. In this sense such collections help to define the field and they are particularly valuable in a rapidly expanding multi-disciplinary environment. The keynote of the approach is rapid publication using IV camera-ready copy provided by the author. This precludes more than a minimal amount of editingl only a few of the most obscure papers can be retyped in the time scale that such an approach makes possible. Also, it does not attempt to include the actual papers as presented at the congress, which often depart from the published text, or to include any assessment of the papers and any consequential discussion. These are valuable but. different areas of interest and the lack of them does not diminish the value of the papers from the point of view of a detailed examination of a topic as it would be normal to establish direct contact with professionals working in an area of interest in order to obtain their latest papers, internal reports and suggestions. The papers for HIE-81, Toulouse, are well up to standard in presenting a var iety of topics in medical informatics from a wide var iety of countries in Europe and beyond. The main focus of the congress is on three specific areas:- 1. Analysis, Evaluation & Improvement of Health Systems 2. Chronic Diseases 3. Informatics, Robotics and the Handicapped rather than the whole range of medical and health care informatics. These are areas where the potential for growth is considerable and it is expected that they will attract considerable interest. It is hoped that this congress will have helped to foster and encourage this interest. During the next decade medical and health care informatics will provide essential tools for the clinical care of patients, for the evaluation of patient care and for the administration and planning of health services. Computer systems will proliferate in more and more interesting medical and nursing applications providing specialised facil i ties to more and more pr acti tioners. These papers represent another milestone in the growth of the subject and offer some further insight into its current development and future potential.
