The data reflected in this report was collected from surveys and focus groups conducted in your local ARH community. These results are being reported along with an update about how we utilized the results from our previous needs assessment from three years ago. The unique design of this CHNA permits an examination of the diverse aspects of each of our 11 ARH communities. The assessment results from each ARH facility demonstrate the desire for individual and community health improvement. These results provide valuable information that will be used for planning purposes, service improvements and community outreach. Special thanks to the CHNA Community Steering Committees in each of our 11 communities for giving of their valuable time and guiding this community health needs assessment process. It is our hope that this assessment will help ARH, in partnership with our communities, to identify respective health concerns, measure the impact of current public health efforts and guide the appropriate use of local resources. We also hope that together, we can improve the health and well-being of the residents of Eastern Kentucky and Southern West Virginia. Sincerely, Joseph L. Grossman ARH President and CEO 2 McDowell ARH Hospital CHNA ARH - Our History In 1956, the United Mine Workers of America (UMWA) and thousands of citizens in the coal communities dedicated the Miners Memorial Hospital Association’s (MMHA) facilities. The system’s hospitals were located in Harlan, Hazard, McDowell, Middlesboro, Whitesburg, Pikeville and South Williamson, Kentucky; Man and Beckley West Virginia; and Wise, Virginia. By the early 1960s, MMHA announced its intention to close some of the hospitals and soon after the Board of National Missions formed a new and independent not-for-profit health system Appalachian Regional Hospitals (ARH)--that purchased the Miners Memorial Hospitals. The health system changed its name in 1986 to Appalachian Regional Healthcare to more accurately describe its far- ranging activities. Today we operate hospitals in Barbourville, Harlan, Hazard, Hyden, McDowell, Middlesboro, Morgan County, South Williamson and Whitesburg, Kentucky, and Beckley and Summers County, West Virginia. ARH has always responded to the changing demands of rural healthcare. Over the years, we have built and acquired new facilities as well as invested in new technology and medical capabilities. Values Excellence, Compassion, Safety, Teamwork, Inclusion, Professionalism 3 McDowell ARH Hospital CHNA CHNA Background Appalachian Regional Healthcare contracted with the Community and Economic Development Initiative of Kentucky (CEDIK) in the fall of 2015 to conduct a Community Health Needs Assessment (CHNA) in accordance with the Affordable Care Act (ACA). The Affordable Care Act (ACA), enacted March 23, 2010, added new requirements that hospital organizations must satisfy in order to be described in section 501(c)(3), as well as new reporting and excise taxes. The IRS requires hospital organizations to complete a CHNA and adopt an implementation strategy at least once every three years. This CHNA was the second prepared by CEDIK for this organization; the first was completed in 2013. Here is an overview of the CHNA process that CEDIK uses based on the IRS guidelines: 4 McDowell ARH Hospital CHNA Since the 2013 Community Health Needs Assessment, we have taken these opportunities to fulfill our promise to the community: www.arh.org/mcdowell We participated in health fairs at Senior Citizens Centers in our area (McDowell/ Minnie Senior Citizens Center, Martin Senior Citizens Center and the Big Sandy Senior Games) to address the issue of Obesity. Our Dietician, Sabrina Hicks, always attends the fairs with us, and discusses proper food choices and portions. To educate We plan to invite the bariatric center to host an education seminar at our facility. We hosted three (3) School Fitness Fairs at South Floyd Middle School, first in the community December 2014, May 2015 and again in December 2015. During these fairs, we of the risks weighed and measured the kids in grades 6, 7 and 8. They visited stations where 1 o#f obesity they did sit ups, push-ups, sit and reach, grip strength, and ran a mile. Their results/ times were written on each child’s card as they visited each exercise. At the end of Y and the high T the event, the cards were collected and they were given a bag with nutritious snacks I pRrevalence and various health information pamphlets. Each student’s results were entered on O a spreadsheet and shared with the school. The students were given a test at the end of the program on what they our children. had learned about their health, the positive effects of exercise and the importance of healthy food choices. At the end of each fitness fair, the children’s BMI was closely monitored and noted, revealing that the majority of the children’s BMI did in fact decrease. A Fitbit was given to both the boy and girl that had the greatest percentage of improvement overall. We also held drawings for 25 - $25 VISA gift cards for the kids that had participated in all three (3) events. 2 # Need for after- Y We already had After Hours clinics on Mondays at both Wayland and Wheelwright hoTurs medical Family Care Clinics until 7:00 pm, but have expanded after hours in our McDowell I seRrvices at our Professional Clinic as well. Our McDowell Clinic is now open from 8:00 am until 8:00 O pm, Monday through Thursday. ouItlying clinics. R P 5 McDowell ARH Hospital CHNA 2013 CHNA priorities for McDowell ARH, continued: We now have more Specialty Physicians that practice in our Specialty Clinic, which is located in the hospital building. We now have a: PM&R physician, Dr. James 3 Hammock, who comes to the hospital once a month. Podiatrist, Dr. David Whiting, # N eed more who visits the clinic twice a month. Oncologist, Dr. Sam Bailey, who comes to the Y STpecialists clinic once a month. Surgeon, Dr. Todd Tom, who comes twice a month and RI performs Stereotactic Breast Biopsies once a month as well. Cardiologist, Dr. (including O Gharad, who now practices in our clinic twice a month. Dr. Sayed Bokhari also CRIardiology) comes to the clinic twice a month. In Dr. Hall’s clinic space, Dr. Koura, P Pulmonologist; Dr. Ahmed, Neurologist; and Dr. Bhagrath, Nephrologist; still come each month as well. “Hosting the Fitness Fairs at South Floyd Middle School was one of the most rewarding events we have held over the past couple of years. The kids were so competitive with each other in the exercises and interested in how much they had grown each time we hosted the event. I was especially proud of how supportive they were for each other when we distributed the prizes for those who participated in the program the entire time. They cheered each other on, and seemed very interested in becoming healthier. Some stated if they had known there would be prizes, they would have tried harder. Approval 35 7 McDowell ARH Hospital CHNA Introduction Appalachian Regional Healthcare (ARH) is a not-for-profit health system serving 350,000 residents across Eastern Kentucky and Southern West Virginia. Operating eleven hospitals, multi-specialty physician practices, home health agencies, HomeCare Stores and retail pharmacies, ARH is the largest provider of care and single largest employer in southeastern Kentucky and the third largest private employer in southern West Virginia.
