Our teachers who inspire and teach us to remain humble in our pursuit of knowledge. Our families and special friends for the continued love and support they have selflessly given. Foreword It gives me immense pleasure to write a foreword for an excellent educative exercise done by Dr. Beena and Dr. Vijaykumar. The management of early breast cancer has always been an area of keen interest for practicing oncologists. The management schema of early breast cancer has dynamically evolved over decades. There has been a paradigm shift in the manage- ment algorithm of early breast cancer, and interestingly there are still several con- troversies in its management. The book compiled, written, and authored by Dr. Beena and Dr. Vijaykumar cov- ers every area from epidemiology, screening, various treatment modalities to rehabilitation. Dr. Beena Kunheri has a special interest in breast cancer, and I am glad she has taken up this task and I am sure it would greatly guide practicing oncologists. Dr. Beena is a leading clinician and academic with several original publications to her credit. Dr. D.K. Vijaykumar is an eminent oncosurgeon and has a special interest in breast cancer surgery. He is one of the pioneers in sentinel node surgery for breast cancer in India. Although we have access to the internet, social media, smart phones, and tablet computers, I still feel that concise information on a dedicated and important topic like early breast cancer, written by experts in that field, would be quite useful and handy for an immediate reference. vii viii Foreword I found it quite comprehensive, covering all aspects of early breast cancer. It provides valuable information and covers all the necessary components of early breast cancer management. I congratulate Dr. Beena and Dr. Vijaykumar on this wonderfully done exercise, and I wish them good luck for their future endeavors too. P. Vijay Anand Reddy, MBBS, MD Director, Apollo Cancer Institute Hyderabad, TS, India Past President, Association of Radiation Oncologists of India Hyderabad, TS, India Preface It is indeed a great pleasure to publish the first edition of this book on Early Breast Cancer. Breast cancer is the most common cancer in women in developed and developing countries like India. In India it accounts for the major proportion of oncologists’ work. The incidence of breast cancer has steadily increased since the 1970s across the globe. However due to the effective implementation of screening programs, early detection, and effective treatment, breast cancer mortality has decreased in developed countries. In developing countries, early detection and improvement in overall survival are yet to be achieved. With the rapidly advancing trends in the treatment and care of women with breast cancer, it has become increasingly difficult for busy clinicians to keep up to date with the latest guidelines and newer approaches to treatment. We have carefully chosen experts from specific fields to author each chapter so that this serves as a comprehensive textbook. The textbook covers a broad range of topics including: • Epidemiology • Prevention • Early detection and diagnosis • Prognostic and predictive markers • Molecular biology • Newer pathological and molecular subtypes • Receptor mechanism • Genetics • Developments in imaging modalities • Surgical treatments • Radiation treatments • Drug therapies • Advances in management including precision and personalized treatment The closing chapter highlights the importance of psychological well-being in cancer treatment. It is included to make physicians aware of the psychosocial issues associated with breast cancer diagnosis and how best to provide psychological sup- port for better outcomes. ix x Preface We believe our book should find a place in all oncology departments as a com- prehensive resource for students and oncologists helping them in their clinical deci- sion making. We also hope our book will be a helpful specialist reference for other disciplines including pathology, radiology, pharmacology, molecular biology, genetics, surgery, and various other specialties. It has been a wonderful opportunity to edit this book. An endeavor like this takes the help of a multitude of individuals without whom this book would not have been possible. We are greatly appreciative of the work of all the authors for the wonderful review and their expertise. In conclusion we want to acknowledge the thousands of women with breast can- cer who have participated in the studies that have provided the information that has helped to advance the field by leaps and bounds in recent years. Kochi, Kerala, India Beena Kunheri Kochi, Kerala, India D. K. Vijaykumar Acknowledgments Pranams to Almighty for making this happen. We are indebted to our teachers, and this work is only a reflection of the inspiration from our teachers who offered us the grounding and foundations. Our pranams to our teachers. We are thankful and indebted to Dr. Pavithran, Head of Dept. Medical Oncology for the invaluable suggestions and support. We are most grateful to all the chapter contributors/authors for agreeing to be part of this project and spending their time in the midst of their busy clinical schedules. Our special thanks to Miss.
