nmograms Save Lives • Mammograms Save Lives • Mammograms i a u v \ c_i / w You Can Help Save Lives by Purchasing a Mammogram Plate Dear Friend: Breast cancer is the second-leading cause of cancer-related deaths among women in the United States, with about one in eight women developing the disease in her life¬ time. Susan Goodman Komen of Peoria was diag¬ nosed in 1978 with breast cancer. Two years later, she lost her battle with this deadly dis¬ ease. With the tragic loss of her sister, Nancy Goodman Brinker established the Susan G. Komen Foundation to educate the public and eradicate breast cancer as a life-threat¬ ening disease. As Secretary of State it is an honor to make available the "Mammograms Save Lives" License Plate. By purchasing a set of the plates, you are helping to provide mammograms and related services to Illinois women who lack the finances for these ser¬ vices. A portion of the proceeds from the sale of the plates goes to the Mammogram Fund, administered statewide by the Susan G. Komen Breast Cancer Foundation—Peoria Memorial Affiliate, Inc. To receive an application for "Mammograms Save Lives" License Plates, please use the attached form. ILLINOIS ST ATE LIBRARY JESSE WHITE Secretary of State El DLD25flDfl 5 Mammogram License Plate Facts Mammogram License Plates are available to Illinois residents who wish to support the mission of the Susan G. Komen Breast Can¬ cer Foundation and help provide mammo¬ grams for women in the state who cannot afford one. The plate design features the pink ribbon symbol for breast cancer awareness on the left side with purple characters on a grey background. "Mammograms Save Lives" and "Susan G. Komen Breast Cancer Foun¬ dation" is printed on the left-hand side of the plate. The plate number is followed by a stacked "CD" suffix (625 ILCS 5/3-643). Mammogram License Plates have a $25 origi¬ nal issuance fee ($10 goes to the Mammo¬ gram Fund), in addition to the standard $78 registration fee. Upon renewal, a $25 fee is charged, $23 of which goes to the Mammogram Fund, plus the standard $78 renewal fee. Plates may be issued to passenger cars, B- trucks or vans weighing 8,000 pounds or less. Vanity and personalized plates are not available. Komen Breast Cancer Foun¬ dation, Inc., was established in 1982 by Nancy Brinker after the loss of her sister, Susan Goodman Komen, to breast cancer at age 36 in 1980. The mission of the founda¬ tion is to eradicate breast cancer as a life- threatening disease by advancing research, education, screening and treatment.
