Erie St. Clair Local Health Integration Network Integrated Health Service Plan 2 Acknowledgements The Erie St. Clair Local Health Integration Network (LHIN) team thanks everyone who helped develop this Integrated Health Service Plan. We are grateful to those who attended and participated in the various engagement mechanisms, sessions, presentations, meetings and surveys throughout our region over the past several months. We are pleased to forward a plan which provides an overview of health services, needs, actions and targets within our LHIN. It is our intention that this plan captures and expresses the best information and solutions to enhanced service delivery available to us.
