Kyle and Emily, please see the information below from Denise regarding resident
and services at another agency. had mentoring services at Children’s Lutheran here in
Springfield that she stated ended in January. moved in with us (LifeHouse) in February. Case
managers from both agencies discussed the case and Children’s Lutheran is going to close
case. Please see the email below.

Just got off the phone with Lutheran Services-Amanda Mcvicker. She is closing their A2A file
with Lutheran Services. Can you let Emily know, also it does look as if the ss number needs to
be corrected on our part. The number ending in is the correct number, looks like we had
a typo.

have on file, according to her drivers license. Does that help?
