P.A. JANSSEN developed etomidate and published this substance in 1971 as "a potent short-acting and relatively atoxic intravenous hypnotic agent in rats". In 1974 after several years of experimental and clinical trial the barbiturate-free hypnotic etomidate (soon marketed as Hypnomidate) was introduced to numerous auditors at the occasion of the IV. European Congress of Anaesthesiologists in Madrid. As the authors extended their essays, the actuality of the present book is given. The conclusion mentions the new solution of etomidate base in 35 % propylene glycol. It is expected that the disadvantageous effects - venous pain during injection and associated involuntary muscle movements - will largely be eliminated by using the new formula of etomidate after premedication of fentanyl. Reneman and P.A.J. Janssen Chemistry Etomidate (R-(+)-ethyl-1-(1-phenylethyl)-1H-imidazole-5-carb- oxylate), which was developed in our laboratories, is a new, potent, short-acting and safe intravenous hypnotic, with the following structural formula: R - (+) Etomidate is supplied as the sulfate (molecular weight; 342.36) and is dissolved in 1.8 mg Na 2 HP0 4 ·12 H20 and 2.2 mg Na H2 P0 4• 1 H20 with 4.2 % glucose in a concentration of 1.5 mg etomidate base per ml (pH about 3.4). General Pharmacology On rapid (2 sec) intravenous injection in rats, etomidate (ED sO = 0.57 mg/kg) is approx. 6 times more potent than methohexitaI (EDSO = 3.S1 mg/kg) and about 2S times more potent than propanidid and thiopental (EDSO'S = 13.4 mg/kg). The safety margin (LDSO/EDSO) in rats is for etomidate 26.0, for methohexital 9.S, for propanidid 6.7 and for thiopental 4.6. The potency and toxicity of etomidate increase slightly with increasing injection rates without affecting the safety margin significantly (JANSSEN et al., 1971, 1975). Etomidate is a pure hypnotic and has no analgesic activity. It has a rapid onset of action and induces hypnosis within one minute. In rats, guinea pigs and dogs, the duration of hypnosis with etomidate is dose-dependent, the duration of the hypnotic effect being doubled when the dose is doubled. With lower body weights
