Barkley, PhD, ABPP, ABCN, is Clinical Professor of Psychiatry and Pediat- rics at the Medical University of South Carolina in Charleston. Barkley and Robin are also the authors of the related parent guide, Your Defiant Teen, Second Edition. v Preface T his clinician’s manual for assessment and family intervention builds on two earlier programs of research and clinical work. First, it represents an extension of Russell Bark- ley’s previous long-term, programmatic work developing an effective program for train- ing parents of defiant children (Barkley, 1987, 1997, 2013a). And, second, it represents our efforts to enhance the efficacy of that program when extended to working with families of adolescents having oppositional defiant disorder (ODD) by modifying those earlier procedures and, more important, combining them with a family therapy program devel- oped by Arthur L. Robin and Sharon Foster (Robin & Foster, 1989). That family therapy program is known as problem- solving communication training (PSCT), and has been shown previously to help in clinical work with families experiencing significant parent– adolescent conflicts. The combination of these programs into a single family training manual is a natural one, given that each targets somewhat different aspects of family functioning in their common goal of improving parent– teen relationships and teen adjustment. Barkley’s pro- gram for defiant children concentrates on giving parents sufficient skills to manage dis- ruptive, defiant, and noncompliant child behavior through controlling the antecedents and consequences of those forms of child misconduct. This exclusive focus on parents’ competence at child management is sensible for families with young children because parents have greater control and influence over young children’s behavior and well-being. In addition, young children are not yet psychologically, socially, or legally autonomous of their parents’ guidance and influence. As children enter adolescence, however, they are increasingly less influenced by their parents’ guidance and their family environment in general. Adolescents are rapidly developing a psychological sense of autonomy and individuation from their parents and families. Thus, to be effective, interventions must focus on the adolescent’s, as well as his or her parents’, need for interactive management, and so must make the adolescent an active participant in any family change program. The PSCT program originally developed by Robin and Foster (1984) does just that: It concentrates on training both the parents and the adolescent in more effective ways of vii viii Preface negotiating conflicts, resolving problems or disputes, developing more effective commu- nication styles, and changing unreasonable beliefs about other family members that may adversely influence specific interactions. Like Barkley’s earlier clinical manual (2013a), this book sets forth a sequence of procedures for training parents in adolescent behavior management skills that Barkley has employed over the past 30 years of clinical research and practice with a variety of behavior- disordered children and adolescents. These procedures have been modified somewhat for the developmental characteristics of adolescence and constitute the first half of the training program (Steps 1–9). Only the parents attend these training sessions, which focus on providing parents with more effective tools for interacting with their teenager and specifically for managing the adolescent’s noncompliant or defiant behav- ior. The family training program of Robin and Foster (1984) comprises the second half of this family treatment program (Steps 10–18); both the parents and the adolescent attend these sessions. The focus shifts toward helping each of the family members develop more effective problem- solving, negotiation, and communication skills with each other, while helping each one to reframe and correct any unreasonable beliefs that may be unearthed in the therapeutic process. The chapters in Part I are adapted from Barkley’s Defiant Children (1997, 2013a) manual for child behavior management training. Additional information on the nature of oppositional and conduct problems in adolescents has also been inserted into this part of the manual. Included in the manual are not only the step-by-step instructions for con- ducting each session of this family training program, but also specific methods for the clinical assessment of adolescents exhibiting defiant behavior or ODD, a condition that is associated in the majority of cases with attention- deficit/hyperactivity disorder (ADHD) and sometimes conduct disorder (CD). Tools for evaluating the nature, diversity, and severity of parent– adolescent conflicts are also provided, as are some tools for evaluating parental psychological adjustment in view of the hereditary nature of ADHD and famil- ial commonality of oppositional behavior. Parts of this program owe much to the stimulating, heuristic, and collaborative dis- cussions we have had over the years with others trained in the original programs, includ- ing Sharon Foster, Charles Cunningham, and Eric Mash. They have been most instru- mental in modifying the program and formulating new components based on current research and clinical practice.
