U ion arrival R/O e 1 R observed P/O Stafiera #621talking to a caller named(cid:9) stated that she 7 ep - o was driving south bound on Potter before Ballard Rd when she observed two vehicles stopped in 18 rt N the north bound lanes.(cid:9) stated that she saw two white males and one was wearing a 1 u 1 m baseball cap.(cid:9) urther stated that both males were yelling at each other and one was . 3 be raising his hands.(cid:9) also stated that she saw a hand gun and heard one shot. 9 r 13C4nliratcd on Reverse 37. CR# RIO asked(cid:9) where this shooting happened at which she responded by pointing her finger towards Potter and Suffield Rd. R/O relocated to said location and canvassed the area. At which time two Des Plaines PD officers Banda #483 and Fitzpatrick #436 blocked north and south bound Potter between Ballard Rd and Church St. R/O located a bullet casing lying on the middle of the roadway by Potter and Suffield Rd. The scene was secured at this time for the preservation of physical evidence. Sgt. Olejarz #89 was notified. P/0 Edwards #243 and P/0 Macugowski #426 arrived on scene. Det. Garcia #947 was also on scene. P/0 Edwards talked to a witness named (cid:9) stated that she was walking with a group on Potter Rd when saw two vehicles stopped in the middle of the street. stated that one was a dark black colored truck and the other was a small vehicle.(cid:9) further stated that she observed one person exited from one vehicle and another person exited from the other vehicle with a pistol.(cid:9) also stated that the person with a pistol said he is a police officer. While RIO was on scene RIO was approached by a off duty CPD officer named Yousif, Daniel, 4th Dist. badge #19663 who told R/O that he was the one who fired a shot. R/O observed no visible injuries on Daniel and Daniel refused any medical attention. RIO observed no signs of impairment on Daniel. Then R/0 told Daniel that his statement will be recorded on body camera. At which Daniel a reed to be recorded. Daniel stated that he was traveling in his Subaru Impreza Illinois registration bearing(cid:9) going north bound on Potter Rd past Dempster St when observed a black colored Dodge pick up truck partial plate(cid:9) with tinted windows trying to race. Daniel stated that the truck cut him off once so he drove around the truck. Daniel stated that the second time the truck cut him off completly so he couldn't go around the truck. Daniel further stated that a male possibly Eastern European, height 605, weight 250 lbs exited the truck and started yelling at him. Daniel stated that the male looked intoxicated. Daniel stated he announced his department but the male started approaching at him so he walked around to keep the distance. Daniel stated that at one point the male came at him aggressively that's when he pulled out his pistol and shot a warning shot towards the ground. Daniel stated that after the shot fired, the male got in his vehicle and sped off northbound on Potter Rd. Daniel stated that he also got back in his vehicle and followed the truck. Daniel stated that he observed the truck traveling at a high rate of speed making a right turn at Church St and passing through the residential area at which time he stopped following the vehicle. Sgt. Olejarz #89, Sgt. Contreras #73 and Det. Lewis #750 arrived on scene. Sgt. Olejarz #89 advised Daniel to make contact with his department representative. CCSPD officers canvassed the area for additional evidence but came with negative results. P/Os also canvassed the area for possible injuries and property damages which came with negative results. P/Os canvassed all the surrounding area for the bullet projectile and came with negative results. FOP representative Jay R. Ryan arrived on scene. The notification to CPD regarding this incident was made by CCSPD dispatch. Lt. Konrad #1 arrived on scene at 2354 hrs. Daniel and his representative went to Lutheran General Hospital for medical evaluation. The CPD Deputy Chief Alexander arrived on scene. ET. Weatherly #775 and ET. Simons #728 arrived on scene at 0040 hrs. The bullet casing was inventoried by ET and the job #81329 was created. The scene was cleared at approximately 0110 hrs. R/O along with ETs, Sgt. Olejarz #89 and Lt. Konrad #1 were relocated to the hospital and Daniel's pistol was inventoried by ET. This investigation was recorded on body camera. Nothing further.
