Salah Mansour Preface Otology and clinical achievements in dealing with middle ear diseases have historically relied on the valuable work of anatomists, beginning with the Renaissance and throughout the revolutionary era of temporal bone micro- scopic dissections. During the last 20 years, thanks to the advanced work and research of otologists, new knowledge of middle ear anatomy had great impact on the understanding of middle ear functions, the management of pathologies, and applicable rehabilitation measures. Most otolaryngology and otology textbooks have however continued to describe the anatomy and embryology of the middle ear with limited detail, thus inadequately serving the needs of learners. Candidates have long empha- sized the need for a single, organized volume covering the latest develop- ments in sufficient detail. Our work is an integrated teaching tool and a source of reference to the available specialty literature. The authors’ purpose is to offer complete, up- to-date coverage of the anatomy of the middle ear, so as to enable those vii viii Preface undergoing surgical training to find and understand both essential and advanced information, and orient clinical decisions and surgical approaches. This book is organized into seven chapters covering the anatomical envi- ronment of the middle ear, its walls, contents and compartments, the mastoid, the facial nerve, and the Eustachian tube. Subchapters and subdivisions sequentially report and illustrate the related embryology and anatomy, as well as the relevant clinical applications. Basic science is thus directly linked to surgical anatomy and clinical experience, based on the firm view that one must master anatomy in order to adequately perform otological surgery. The anatomy of the middle ear is explained with numerous color pictures and helpful figures. The book correlates clinical situations to the anatomical basis of diseases and explains the pertinent embryology in order to highlight the buildup of normal anatomy and developmental anomalies. The inclusion of carefully selected CT scans will assist in the reading of normal anatomy and the identification of pathological findings. This first edition is the result of close collaboration among very good friends who share a strong devotion to medical teaching. It is a tribute to my colleagues’ patience and their dedication to selecting the best materials in respect of each topic. I owe them my warmest and most sincere recognition, and many thanks. You have to gather together material and men, then, instead of hiking, write for a very long time. As the writing goes on, the completion of the book seems all the more distant, just as in climbing the summit seems always farther away. To succeed in such an undertaking, you need a guide, someone who controls the situation and shares his enthusiasm. Salah Mansour has been this unquestioned leader of the expedition. I am but the second in command, there to assure and reassure in case of difficulty. We met fairly recently, during Salah’s visit to Marseille in 1999. His ele- gance, his culture, and his teaching are captivating. He is surrounded by bril- liant students of whom Hassan is one of the most faithful. One of the main difficulties in mastering otology is not only its complexity but also the diversity of sources that have to be referred to, so as to learn about and understand this specialty thoroughly. Salah and I wasted too much time at the beginning of our career searching for good textbooks, the right infor- mation. Hence, our desire is to overcome this obstacle and bring together in one sole reference book the necessary fundamentals to become an otologist. Thus, this book on the anatomy of the ear, with its detailed descriptions, is completed and illustrated by embryology, pathology, and surgery from our own personal experience. The sole purpose of this book is to fix the boundar- ies within which the rules of our surgical specialty should evolve. It is up to each practitioner to express himself/herself within these boundaries. Salah Mansour asked me to participate first in editing the chapter on the external auditory canal, then for the entire book. I thank him for this expres- sion of his trust and friendship.
