Overview and Purpose Addison Gilbert and Beverly Hospitals have a long history of engaging with and supporting communities in its service area • Community Benefit Committee • Community grants and initiatives with community agencies addressing: – Health education, prevention, chronic disease management, elder health, domestic violence, etc. Refined state and new federal regulations now require a more robust community health needs assessment, community engagement, strategic planning, and reporting • Massachusetts Attorney General Community Benefit Guidelines • Federal Internal Revenue Service (IRS) Community Benefit / Form 990 Requirements State and federal regulations now require that not-for-profit hospitals: • Conduct a comprehensive community health needs assessment every three-years • Conduct the assessment in partnership with local public health departments and ensure that the community has the opportunity to provide input • Identify community health priorities and key target populations most at-risk • Develop and implement a detailed community health improvement plan that addresses identified needs and that includes clear goals, objectives, and outcome measures • Track, monitor, and report on the impact of the hospital’s community health improvement plan • Report findings of the Community health needs assessment and the community health improvement plan to the public at-large As a result Addison Gilbert and Beverly Hospitals hired John Snow, Inc. a public health and health care consulting firm to assist them with a community health needs assessment and facilitate the development of a community health improvement plan 2 Community Health Improvement Plan March 24, 2014 Section II – Approach & Methodology Review of Approach Comprehensive assessment of need, service system strengths and Addison Gilbert and Beverly Hospital’s strategic assets/resources 3 Community Health Improvement Plan March 24, 2014 Review of Methodology Three-phased approach completed in November 2012 that captured quantitative and qualitative data, involved public health, social service, and health care providers, and engaged the communities- at-large 4 Community Health Improvement Plan March 24, 2014 Section III - Community Health Improvement Plan Guiding Principles of Planning Process • Develop shared agenda within & across hospital, health department and other community partners • Build-on existing hospital, health department and community-based initiatives • Develop targeted, community-based, linked, collaborative, well-integrated efforts among community partners • Develop shared evaluative metrics • Develop infrastructure to guide & monitor activities Core Community Benefit /Community Health Strategies • The following strategies were identified by the steering committee to respond to the health status issues and community needs identified by the assessment for the low income, racial/ethnic, minority and older adult target populations • There is synergy between these strategies and Lahey Health’s (Addison Gilbert and Beverly Hospital’s) existing business strategy 7 Community Health Improvement Plan March 24, 2014 Section V – Community Health Improvement Plan Dashboard Community Benefit Infrastructure The following enhancements in Lahey Health’s (Addison Gilbert and Beverly Hospitals) community relations/community benefit infrastructure will be required for the organization to develop the necessary community health partnerships Priority Areas and Major Goals The following goals will be implemented by Addison Gilbert and Beverly Hospitals over a three-year period Community benefit activities will be implemented in close partnership with various community agencies, including local health departments, councils on aging, YMCAs/Boys & Girls Clubs, and other health/social service agencies. These strategies will be implemented by Lahey Health’s (Addison Gilbert and Beverly Hospital’s) community relations department in close collaboration with a number of other administrative and clinical departments depending on the specific strategic area Priority Area 1: Elder Health Enhance access to health and wellness services through improved transportation Promote general health and wellness Improve chronic care management Reduce inappropriate hospital readmissions & enhance care transitions, drug management, and follow-up Reduce falls in elders Decrease depression and social isolation Priority Area 2: Behavioral Health (Mental Health, Substance Abuse, and Domestic Violence) Promote education and awareness of behavioral health, wellness, and engagement in primary care Improve coordination of existing mental health and substance abuse services across community partners Increase access and integration of mental health and substance abuse (MH/SA) services in primary care 8 Community Health Improvement Plan March 24, 2014 Promote emergency department diversion/SBIRT programs Increase awareness and screening for domestic violence (DV) throughout the system Priority Area 3: Access to Care Increase access for uninsured residents in order to promote access to health care services Improve coordination between Beverly Hospital’s and Addison Gilbert Hospital’s primary care / medical specialty care and other community health and social service providers Increase capacity of primary care and medical specialty care in Lahey Health’s outpatient clinics & practices Increase access and integration of mental health and substance abuse (MH/SA) services in primary care Develop and expand idealized primary care and care management Priority Area 4: Wellness, Disease Prevention, and Disease Management Primary Prevention Promote general wellness, behavioral change, and engagement in primary care Increase healthy living, physical activity, and healthy eating Secondary Prevention Increase screening for chronic diseases (including cardiovascular disease, diabetes, and cancer) Promote Stamford Model Chronic Disease Self-Management program Tertiary Prevention Develop and expand idealized primary care and care management Promote mental health and primary care integration 9 Community Health Improvement Plan
