Preface There are many people to whom I am very thankful for their help and sup- port during the writing of this monograph. As this is a revision of my Ph.D. thesis, my first thanks must go to my supervisor, Professor Francis Watson. I am very grateful for his generosity, wise guidance, enthusiasm and good humour.
