De Salles Department of Neurosurgery David Geffen School of Medicine at UCLA 10495 Le Conte Ave., Suite 2120 Los Angeles, CA 90095 USA Foreword In previous decades the notion of multidisciplinary care was embraced to bring increasingly complex treatment modalities safely and expeditiously to patients. Stereotactic radiosurgery is exemplary of such a mutual effort. In this regard, the paradigm of bringing together the neurosurgeon, radiation oncologist, diagnostic radiologist, and medical physicist into collaboration served the development of the medical science and our patients well. We now enter an era of an enhanced and more sophisticated understanding of anatomy, molecular biology, radiation biology, genetics, and imaging science as it relates to stereotactic based radiation treatments. The technology we use has contin- ued to improve in its fexibility, accuracy and precision giving us the ability to maxi- mize therapeutic beneft while minimizing morbidity. This progress, however, comes with concomitant increase in complexity of structure and process. Stereotactic radiation techniques such as stereotactic radiosurgery (SRS) and ste- reotactic body radiation therapy (SBRT) serve as a paradigm for breaking down the silos of traditional department-based medical disciplines by setting up integrated practice units for the beneft of the patient. We come together out of necessity to address the complexity of the processes. Barriers to this integration remain, but will continue to be addressed and overcome. The techniques developed by the collabora- tion now allow for the lessons learned from SRS in the brain to be applied to the spine, pancreas, prostate, kidney, and with motion management, to the lung and liver. The application of SRS and SBRT now abound in curative as well as palliative settings – and with new notions of the treatment of oligometastasis, it shows promise to convert heretofore incurable cancer into a chronic disease. Even more exciting, the frontiers of research afford the possibility of using radiation in the SRS/SBRT context to manipulate signaling pathways of cancer cell growth and immune response (Danger Theory) for the beneft of the patient. Radiation biologists are developing a new understanding of the nuances of the radiation interaction with cancer- ous and normal tissues to achieve genetic changes that can be translated to enhance apoptosis, expedite healing, prevent excessive repair reaction of normal tissue, and even stimulate the production of neurotransmitters in cases of neurodegenerative diseases. The chapters ahead review the progress made and set out possibilities for the future. The future of this complex discipline, stereotactic radiation, is indeed bright. Los Angeles, CA, USA Michael L.
