IN NORTH COLLINS, ERIE COUNTY, NY. DISPATCHER DETERMINED FROM SCADA AND OTHER PHONE CONTACTS, THAT LINE KNY WAS EXPERIENCING GAS LOSS. A REGULATOR CREW WAS DISPATCHED TO CLOSE BLOCK VALVE NCE-5 IN NORTH COLLINS AND SEND A SUPERVISOR TO NORTH BOSTON STATION TO CLOSE VALVES BNE-193 AND 195.VALVES BNE-193 & 195 WERE CLOSED AT 12:12 PM AND THE VALVE NCE-5 WAS CLOSED AT 12:32. AT THAT TIME THE LINE WAS ISOLATED.WHEN THE SITUATION WAS UNDER CONTROL IT WAS DETERMINED THAT A RUPTURE HAD OCCURRED ON LINE KNY APPROXIMATELY 641’ SOUTH OF LOWER KNOLL RD, AND NORTH OF NEW OREGON RD. FURTHER INVESTIGATION REVEALED THAT NO PERSONAL INJURIES OR DEATHS RESULTED FROM THE INCIDENT. THERE WAS SOME PROPERTY DAMAGE AT 9999 NEW OREGON RD. THE PIPELINE WAS PAIRED BY REPLACING APPROXIMATELY 4 FEET OF 20” OD, 0.375" WALL, X-52 GRADE, PRETESTED STEEL PIPE. PART G: NARRATIVE DESCRIPTION OF FACTORS CONTRIBUTING TO THE EVENT - CONTINUED:INVESTIGATION UPDATE: THE FAILED SECTION WAS DELIVERED TO BATTELLE, COLUMBUS OHIO FOR A COMPLETE METALLURGICAL AND FAILURE ANALYSIS TO DETERMINE THE ROOT CAUSE OF THE FAILURE. BATTELLE CONCLUDED:"THE ORIGIN FOR THE FAILURE WAS IDENTIFIED AS A PAIR OF PITS COINCIDENTALLY LOCATED WITHIN THE CROWN OF THE PIPE IN THE VICINITYOF MAXIMUM AXIAL STRESS. THE UNUSUAL LOCAL AXIAL LOADING ACTED ON THIS PAIR OF COINCIDENTALLY LOCATED PITS CAUSING FAILURE, WHICH WAS FACILITATED BY WINTER TEMPERATURES WORKING IN CONJUNCTION WITH THE INHERENTLY HIGH DBTP' LEADING TO REDUCED FRACTURE TOUGHNESS. They were hired to excavate the contaminated soil area. Pipe cut off at roadway and liquid drained and contained. End of pipes capped. 6/10/08 − Visited site and inspected area. Old pipeline was previously cut off about 50 feet off highway. Area is a wet land drainage area. Soils around piping appear to be silty and water at 4−5 foot depth. Tight soils. About a 15 x 20 foot area to be excavated of any petroleum contaminated soils. One composite soil sample to be taken after excavation to confirm cleanup. Env. Approx 24,000 gallons of kerosene is "unaccounted for" and is assumed to be under the plant. Kerosene has shown up in a manhole in a nearby service road. 03/01/10 Performed site visit this morning. Underground product supply line from two(2)million kerosene AST to Gas Turbine Unit(GT−3) leaked and released unknown quantity of kerosene to subsurface. After inventory reconciliation,the quantity estimated to be approx 24,000 gallon. The underground pipe is about 150−160 ft in length. Excavation on the pipeline where kerosene bubling up the surface started this morning.Clean venture began recovering the free product from the containment moat of the 2 mil gallon AST. Shore line has been boomed off, as precaution. Existing MWs on the bulkhead is monitored daily. No sign of any product in the monitoring wells is found. December 15, 2015 Key Spills Summary Page 10 secondary containment for tank KO−1 where water and product leaking into the dike from cracks and an abandoned pipe penetration, in the ConEd manhole on the 36th Avenue service road, in the excavation at the north end of GT 3 where product is seeping out of the soil.Preparations are underway to offload the KO−1 tank.(sr) 03/09/10 As of 03/08/10, approx. 2575 gal of kerosene recovered.On 3/9 more excavation pits along the northern fence will be dug to determine the east−west boundaries of the product plume.In the Con Ed manhole on the 36th Ave service Rd, product was no longer entering through a conduit buried approx 3 ft below the surface.Investigation of the manhole identified a 2" drain hole at the bottom where product was visible. It was inferred that product was on the groundwater in this area.A test pit to the water table was excavated at the north boundary of Ravenswood in the vicinity of the Con Ed manhole. Visible product was evident in the test pit. Two more test pits were excavated at the north boundary of Ravenswood−one about 30’ west of the ConEd manhole and the other about 50’ east of the manhole. Both test pits evidenced product at the water table. The transfer of Kerosene from tank KO−1 to the barge commenced on 03/06.(sr) 03/22/10 Email from Ravenswood. "The following is the status of the kerosene spill (NRC #932394, DEC #0912294, DEP #184991838) at the Ravenswood Generating Station as of Monday, March 22, 2010: * There has still not been any visible product on the East River to date. * On Monday 3/15 we disbanded the internal incident command system. we had set up at Ravenswood. After over two weeks of investigation, we felt confident we had determined the approximate extent of the spread of contamination. There was no longer an immediate risk of the product contaminating the East River. This was no longer an emergency response, and we transitioned into a recovery and remediation project. *On Tuesday 3/16 and Wednesday 3/17 we conducted seismic refraction testing to map the bedrock contours to the north of the spill location. We expect to have results of that testing back this week, to help us determine the flow path of product away from the leak site. * On Thursday 3/18 we commenced drilling monitoring/recovery wells. Three wells were installed through Friday 3/19. A total of seven 4" wells will be installed on site using the sonic drill rig, with some additional 2" wells installed using a Geoprobe rig along the fence line. * On Friday 3/19 we received the NYCDOT permit to excavate test holes in the 36th Ave. service road. We will be working with the contractor for the bridge refurbishment project to complete those test holes this week, if possible. * The abandoned 12" fuel oil line to the Rainey Tanks was opened on Friday 3/19. It was determined that this was not acting as a conduit for kerosene, as it already contained old No. 6 oil from its abandonment in 1965. Under a separate project, this line will be cleaned, detached from the kerosene unloading line, and properly abandoned in place with grout. * As developments are happening more slowly now, I will be sending these updates less frequently probably once or twice a week. I will keep you updated should any important developments arise."" 04/30/10 As per email on 04/29/10, there has still been no evidence of product release to the East River.Four wells have been drilled and installed on the 36th Ave. service road. The last two wells were completed on 4/28.One more test pit will be dug in the 36th Ave. service road to clarify the eastern boundary of the product.TranCanada built three sumps inside the KO−1 containment to facilitate recovering the product that seeps in through the wall.
