Williams #847, Area Central Detective Division. This report is encapsulated in essence and not verbatim unless otherwise indicated. This report should be read in conjunction with all other related reports also under RD#503123 and RD#503071. R/D relocated from AREA CENTRAL to the 009th District and was met by Lt. BEAZLEY #182. The facts of the case were related as so per the case reports: BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780, responded to an OEMC call at 3651 S Hoyne Ave. BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780 spoke with the Victim/Complainant who related JARDINAS, Andy (OFFENDER) broke the rear window of his residence. JARDINS, Andy was placed into custody on signed complaints by BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780. BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780 suspected OFFEDNER to be intoxicated and under the influence of drugs. BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780 requested CFD. AMB #69 arrived and transported JARDINAS, Andy to St. Anthony's Hospital. While at St. Anthony's Hospital to be treated, JARDINS, Andy was striking his head against the hospital wall. JARDINS, Andy was treated and released by Dr. Ephriam GRIMES and transported back to the 009th District by BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780. BT 911R, P.O. CORONA #7852 and P.O. ARROYO #5780 arrived at the 009th District and escorted JARDINAS, Andy to the holding cell located in the 009th District processing area with his hands cuffed behind his back. While in the holding cell, JARDINAS, Andy was able to maneuver in such a manner to where he was able to have his hands in front of him, still handcuffed. JARDINAS, Andy then began to strike the glass of the holding cell with the handcuffs causing the glass to shatter/spider web. P.O. CORONA #7852 walked over to the holding cell to stop JARDINAS, Andy from damaging the glass. P.O. CORONA #7852 opened the holding cell door via key card locking system. When the door opened, JARDINAS, Andy stepped towards P.O. Corona #7852 in an aggressive manner while raising his hands that were handcuffed in front of him. P.O. CORONA #7852 pushed JARDINAS, Andy away causing OFFEDNER to fall backwards and striking his head on the floor. JARDINAS, Andy sustained a lump to the back of his head. From Mercy Hospital, JARDINAS, Andy was transported to Cook County Hospital. R/D relocated to Cook County Hospital for a status on JARDINAS, Andy. R/D observed Bt. 973, P.O. KELLY 12155 and P.O. MALLOY #17010, guarding JARDINAS, Andy in the emergency room. R/D went to the nurses' station and was told by the attending medical staff that JARDINAS, Andy sustained Intracranial Hemorrhage (ICH) and Subdural Hematoma (SDH), both being skull fractures with brain bleeding. It was related to R/D that JARDINAS, Andy is in critical condition and will be closely monitored but should be ok. R/D relocated back to the 009th District. Commander GODSEL arrived. Moments later Deputy Chief Ryan arrived along with IPRA Investigators, GOODWIN and REYNOLDS. All listed personal were present at this time. R/D directed the Evidence Tech P.O. MANGUERRA #14098 to the lockup. At the lockup, R/D directed the Evidence Tech P.O. MANGUERRA #14098 to process the holding cell. Blood was observed on the wall, floor and sliding door. Also observed was the glass next to the sliding door damaged and spider webbed. Pictures and DNA swabs were requested. R/D relocated to St. Anthony's Hospital. R/D spoke with Hospital Officer RIOS #202. Hospital Officer RIOS #202 related there is video footage of the Emergency Room but the individual who has access to it is not working. Hospital Officer RIOS #202 related Mike Suller is the one who is responsible for the video footage and he works Monday thru Friday, 6am-3pm. Hospital Officer RIOS #202 related if R/D was unable to get ahold of him for some reason, R/D should contact his assistant, Jose Rodriguez who also works Monday thru Friday, 6am-3pm. R/D attempted to interview the nurses and Doctor who treated JARDINAS, Andy but was advised that their shift had ended and all had left. R/D was told that JARDINAS, Andy was treated in room #8 of the Emergency Room. R/D viewed the video footage from the 009th District lockup. R/D did observe P.O. CORONA #7852 walk JARDINAS, Andy to the holding cell. JARDINAS, Andy was put into the holding cell with his hands handcuffed behind his back. R/D observed JARDINAS, Andy maneuver in a manner that he was able to put his hands in front of him still being handcuffed. R/D then observed JARDINAS, Andy, with his hands handcuffed in front of him, raise his hands and strike the glass. R/D observed P.O. CORONA #7852 and P.O. ARROYO #5780 walk over to the sliding door of the holding cell. As the door was opened, R/D observed JARDINAS, Andy advance towards P.O. CORONA #7852. R/D then observed P.O.CORONA #7852 push JARDINAS, Andy away from him. Let it be noted that the corner of the holding cell is constructed of mason block and a clear direct visual path is not obtainable. R/D did observe the OFFEDNER moments later up and walking around in the cell. S E V On 06 November 2016, R/D went to Cook County Hospital to interview JARDINAS, Andy. R/D read JARDINAS, Andy his Miranda I T rights @ 01:50Hrs. R/D asked JARDINAS, Andy what had happened. JARDINAS, Andy related he was drinking Miller Lite and A Stella Beer. JARDINAS, Andy related he remembers being lost and trying to get home. R/D asked JARDINAS, Andy if he R R remembered breaking the window @ 3651 S. Hoyne. JARDINAS, Andy related NO.R/D asked JARDINAS, Andy if he remembered A being arrested. JARDINAS, Andy related NO. R/D asked JARDINAS, Andy if he remembered being taken to the hospital. N JARDINAS, Andy related NO. R/D asked JARDINAS, Andy if he remembered striking the glass in the 009th District, damaging the glass and causing it to shatter/spider web. JARDINAS, Andy related NO. R/D asked JARDINAS, Andy if he remembered approaching P.O.CORONA #7852 with his hands cuffed in front of him and raising his hands up toward P.O. CORONA #7852 in an aggressive manner. JARDINAS, Andy related NO. JARDINAS, Andy related to R/D that all he remembers is drinking the beer and then waking up in the Hospital. R/D asked JARDINAS, Andy if there is anything he can remember from that night. JARDINAS, Andy related he does not remember anything at all. On 08 November 2016, R/D spoke with Mike Suller, head of security of St Anthony's hospital. R/D asked Mr. Suller about possible video footage in the Emergency Room. Mr. Suller stated that there is footage of the waiting room but no footage of individual rooms where patients are treated. On 11 November 2016, R/D spoke with P.O. ARROYO #5780 regarding what transpired on 05 November 2016 involving JARDINAS, Andy. P.O ARROYO #5780 related to R/D that he and P.O. CORONA #7852 were working Bt. 911R. They responded to an OEMC call of a Suspicious Person. When they arrived they spoke with (VICTIM/COMPLAINAT) who stated that JARDINAS, Andy had broken the rear window of his residence. Complaints were signed and JARDINS, Andy was placed into custody. While on scene, P.O. ARROYO #5780 and P.O. CORONA #7852 observed JARDINAS, Andy not to be in the right state of mind, possibly under the influence of alcohol and/or drugs. A request was made on scene for CFD and AMB #69 arrived. AMB #69 transported JARDINS, Andy to St. Anthony's hospital. P.O. ARROYO#5780 related to R/D that while P.O. CORONA #7852 and JARDINS, Andy were in St. Anthony's Hospital, P.O. ARROYO #5780 stayed in the squad car to complete a police report. JARDINS, Andy was discharged and transported to the 009th District for processing. P.O ARROYO #5780 related that when they arrived at the 009th District, JARDINS, Andy was escorted to the holding cell located in the 009th District processing area. P.O. ARROYO #5780 related JARDINS, Andy began to bang on the glass of the holding cell with the hand cuffs that were now in front of him causing the glass to shatter/spider web. P.O. ARROYO #5780 and P.O. CORONA #7852 relocated to the door of the holding cell. P.O. ARROYO #5780 related he observed JARDINS, Andy approach P.O. CORONA #7852 at which time P.O. CORONA #7852 pushed JARDINS, Andy away from himself causing JARDINS, Andy to fall to the ground. P.O. ARROYO #5780 related to R/D that the handcuffs were removed and JARDINS, Andy had got up and sat on the bench. P.O. ARROYO #5780 and P.O. CORONA #7852 exited the holding cell. P.O. ARROYO #5780 related JARDINS, Andy began to bang on the holding cell glass again. P.O. ARROYO #5780 related to R/D that he went to the holding cell and handcuffed one hand to the bar that is attached to the wall. P.O. CFD was requested. P.O. ARROYO #5780 related he observed blood on the wall of the holding cell. P.O. ARROYO #5780 examined JARDINS, Andy and observed blood on the rear of JARDINS, Andy's head. AMB #19 arrived and transported JARDINS, Andy to Mercy Hospital. On 11 November 2016, R/D went to St Anthony's Hospital and interviewed Both stated that JARDINS, Andy was treated in Emergency Room #8 on 05 November 2016. Both stated that they remember JARDINS being very confrontational and loud. Both stated that they observed JARDINS, Andy in Emergency Room #8, via the monitor at the nurses' station, banging his head off the wall and kicking the door. Both related JARDINS, Andy declined medical treatment and he wanted his lawyer, JARDINS, Andy was eventually seen by Dr. Grimes and was discharged from St Anthony's into police custody. On 12 November 2016, R/D spoke with P.O. CORONA #7852. R/D asked P.O. CORONA #7852 what had happened on 05 November 2016 regarding JARDINS, Andy. P.O. CORONA #7852 related that P.O. ARROYO #5780 and himself were working BT. 911R. P.O. ARROYO #5780 and himself responded to an OEMC call of a suspicious person at 3651 S Hoyne and spoke with the (VICTIM/COMPLAINANT.) It was related that JARDINS, Andy had broken a window in the rear of his residence. JARDINS, Andy was placed into custody on signed complaints. While on the scene, P.O. CORONA #7852 related JARDINS, Andy was not making any sense when he was talking, his lips were all white and he was "Jaw-Jacking" which lead P.O. CORONA #7852 to believe that JARDINS, Andy was possibly under the influence of alcohol and/or drugs. CFD was requested. AMB #69 arrived on scene and transported JARDINS, Andy to St. Anthony's Hospital. While at St. Anthony's Hospital, P.O. CORONA #7852 related they were in Emergency Room #8 and JARDINS, Andy was banging his head against the wall and kicking the door. P.O. CORONA #7852 told JARDINS, Andy several times to stop it. Dr. Ephriam Grimes entered the room and told JARDINS, Andy that if he continued that he would need to be sedated. JARDINS, Andy calmed down and was able to be seen by Dr. Ephriam Grimes. JARDINS, Andy was discharged from St. Anthony's Hospital and transported to the 009th District for processing. P.O. CORONA #7852 stated that P.O. ARROYO #5780 remained outside the Hospital in the squad car completing a report during these events. P.O. CORONA #7852 related when they arrived in the 009th District, JARDINS,Andy was escorted to the holding cell in the 009th District processing area. JARDINS, Andy was put into the holding cell with his hands cuffed behind his back. P.O. CORONA #7852 S went to complete his paperwork when several moments later he heard banging on the holding cells glass and observed JARDINS, E Andy striking the glass with the handcuffs that were now in front of JARDINS, Andy. P.O. CORONA #7852 related he went to the V I holding along with P.O. ARROYO #5780. P.O. CORONA #7852 related he activated the key card locking system to unlock the T A door and when the sliding door opened, JARDINS, Andy approached P.O. CORONA #7852 in an aggressive manner raising his R hands that were handcuffed, in front of him, in an attempt to strike P.O. CORONA #7852. P.O. CORONA #7852 related that when R JARDINS, Andy approached and saw JARDINS, Andy begin to raise his hands to strike him, P.O. CORONA'S #7852 natural A N reaction was to defend himself by pushing JARDINS, Andy away. P.O. CORONA #7852 related JARDINS, Andy fell backwards and struck his head on the ground. P.O. CORONA #7852 removed the handcuffs and made sure JARDINS, Andy was ok. P.O. CORONA #7852 related JARDINS, Andy related he was ok and sat on the bench. P.O. CORONA #7852 related he then returned to complete his paperwork. P.O. CORONA #7852 related moments later, JARDINS, Andy began to bang on the holding cell glass again. P.O. CORONA #7852 related that P.O. ARROYO #5780 went to go see what was going on. It was related to P.O. CORONA 7852 by P.O. ARROYO #5780 that CFD was request and that P.O. ARROYO #5780 observed blood on the holding cell wall and blood on the back of JARDINS, Andy head. AMB #19 arrived and transported JARDINS, Andy to Mercy Hospital. On 16 November 2016, R/D went to St. Anthony's Hospital and interviewed Dr. Ephriam Grimes. Dr. Grimes related he remembered treating JARDINS, Andy on 05 November 2016 in Emergency Room #8. R/D asked Dr. Grimes if he remembered JARDINS, Andy banging his head on the wall in Emergency Room #8. Dr. Grimes related yes he was banging his head on the wall but he kicked the door more. Dr. Grimes related he entered Emergency Room #8 and told JARDINS, Andy that he would use sedation if he didn't calm down. Dr. Grimes related that JARDINS, Andy was saying he didn't want medical treatment and that he wanted his lawyer. Dr. Grimes also related to R/D that JARDINS, Andy asked for his handcuffs to be loosened. R/D asked Dr. Grimes if JARDINS, Andy's handcuffs were loosened and Dr. Grimes related yes, the Officer loosened them up. Dr. Grimes also related that JARDINS, Andy calmed down and he was able to take JARDINS, Andy's vitals and give a quick exam. JARDINS, Andy was discharged into police custody. R/D request this case be CLEAR CLOSED by ARREST/PROSECUTION. Det.
