Eligibility is determined by a Home Health professional. Some services are available to home-bound clients only. Cost - eS riv ec s rp oiv ed d rf ee o f ahc reg to eil ig elb Home Heatl h ilc ent s inulc ed : case management, occupatonal and physical therapy, nutriton counselling and educaton, and health services for adults with developmental disabilites. People who are considered to be palliatve by their doctor and who are in the last six months o f il ef are not ahc reg d of r oh me us op rt services. A team of health care professionals, including primary care doctors, a nurse specializing in seniors care, a geriatri- cian and geriatric psychiatrist work alongside patents, their family members and family doctors to ensure seniors are supported in their ongoing health needs.
